Doctor Montagu Lomax was a retired General Practioner, whose service in English lunatic asylums during the First World War inspired him to write The experiences of an asylum doctor: with suggestions for asylum and lunacy law reform. Published in 1921, the book acted as a catalyst for lunacy reform and stimulated improvements in the mental health services in the United Kingdom. Lomax spent the remainder of his retirement campaigning for lunacy reform. He suffered financial and personal hardship following the publication of the book and was castigated by his own profession. On the centenary of the publication of Experiences, this article explores the background and motivation of a remarkable man.

Background:We evaluated the quantitative DiaSorin Liaison SARS-CoV-2 antigen test in symptomatic and asymptomatic individuals consulting their general practioner (GP) during a period of stable intense virus circulation (213/100,000 habitants per day). Methods:Left-over RT-PCR positive (n=204) and negative (n=210) nasopharyngeal samples were randomly selected among fresh routine samples collected from patients consulting their GP. Samples were tested on Liaison XL according to the manufacturer’s instructions. Equivocal results were considered positive. Results:Overall sensitivity and specificity of the Liaison antigen test compared to RT-PCR were 67.7% [95% confidence interval (CI): 60.9%-73.7%] and 100% [CI: 97.8%-100%]. Sensitivity in samples with a viral load ≥105, ≥104 and ≥103 copies/mL was 100% [CI: 96.3%-100.0%], 96.5% [CI: 91.8%-98.7%] and 87.4% [CI: 81.3%-91.5%], respectively. All samples ≤103 copies/mL were antigen negative. The ratio of antigen concentration to viral load in samples ≥103 copies/mL was comparable in symptomatic and asymptomatic individuals (p=0.58). The proportion of RT-PCR positive participants with a high viral load (≥105 copies/mL) was not significantly higher in symptomatic than in asymptomatic participants (63.9% [CI: 54.9%-72.0%] vs. 51.9% [CI: 41.1%-62.6%], p=0.11), but the proportion of participants with a low viral load (<103 copies/mL) was significantly higher in asymptomatic than in symptomatic RT-PCR positive participants (35.4% [CI: 25.8%-46.4%] vs. 14.3% [CI: 9.0%-21.8%], p<0.01). Conclusions:Sensitivity and specificity in samples with a viral load ≥104 copies/mL was 96.5% and 100%. The correlation of antigen concentration with viral load was comparable in symptomatic and asymptomatic individuals.

AimThe technology of online and mobile-based symptom checkers is developing on a background of unprecedented demand for General Practioner (GP) and medical services. This study was performed to assess the performance of popular ‘artificial intelligence’ symptom checkers currently available to the public and evaluate their accuracy as a screening tool for head and neck cancer symptoms.MethodWe selected three of the most popular and previously compared mobile application (app) symptom checkers available including Babylon (London), Your.md (London) and Ada (Ada Health GmbH, Munich). A recent Pan-London Suspected Cancer Referral Guide was obtained and distilled into a list of qualifying symptoms for referral to secondary care on a 2-week wait cancer pathway. A generic patient background was devised and each symptom was passed through the selected symptom checker apps. Differential diagnoses and recommended triage times were compared.ResultsOverall a cancer diagnosis was suggested possible in the differential diagnosis in 33% of symptoms. As a screening tool, Babylon was the most accurate for including a potential cancer diagnosis in the differential diagnosis, with a sensitivity of 45% cases, Ada with 32% and Your.md 23%. Ada attempted the most diagnoses, able to provide a differential diagnosis in 95% scenarios, but was also the most risk averse triage assessor, with 55% outcomes recommending medical advice immediately or within hours.ConclusionThe ability of modern symptom checkers to promote health awareness and self-checks is promising, but we have identified a number of areas of potential improvement. At present the sensitivity of the tested symptom checker apps remains low for head and neck cancer.

Introduction: The article describes the clinical case devoted to the features of the clinical course of Churg-Strauss syndrome (eosinophilic granulomatous with systemic necrotizing vasculitis). Churg-Strauss syndrome is a disease that is rarely diagnosed because of presenting by high-variability clinical syndromes, that causes difficulties in diagnostic process, especially on the first visit of patient to the general practioner. The aim of the article is to show the clinical course feautures of the Churg-Strauss syndrome in the time of disease progression, as well as pecullarities and possibilities of in-time diagnosis of this disease. Clinical case: It was described a clinical case of Churg-Strauss syndrome in a 46-year-old woman, presenting with successively appeared signs of the disease that resulted in some difficulties of making in-time diagnosis. Conclusions: As Churg-Strauss syndrome is a low-spread disease presenting with high-variety of clinical signs and despite of the presence of clear diagnostic criterias Churg-Stross syndrome was diagnosed with delay in this clinical setting. General practitioner should remember that if the disease debuts by the signs of pneumonia with further appearance of signs of bronchial obstruction and eosinophilia, its necessary to perform diagnostic approaches with the aim to carry out the presence of the Churg-Strauss syndrome.

Peripheral causes are those lesions of the end organs and vestibular nerve, and it includes Menieresdisease, Labyrinthitis,Acoustic neuron, ototoxicvertigo, Syphilis(vertigo with deafness) and Benign paroxysmal positional vertigo (BPPV), Vestibular neuronitis(vertigo without deafness).Central causes are lesions of brainstem and central connections, posterior inferior cerebellar artery syndrome,vertebrobasilarinsufficiency, Basilar artery migrane,Tumours of the brainstem and fourth ventricle,Epilepsy,Cervical vertigo.

<p>Recurrent Aphthous Stomatitis (RAS) is a disorder in the oral cavity, with a characterized symptom as ulceration, recurrent and very painfull. The etiology is idiopathic, with multifactorial predisposition. Exfoliative cheilitis is a persistent lesion on the lip, with a characterized cracking and desquamative, with crustae and inflammation. An elderly male (72 yrs) suffered with ulcer on his oral cavity, cracking lips and pain on both of his cheeks, skin, since 5 years ago. The pain is recurrent. On the clinical examination, there were some desquamation, both on the skin and vermilion border, whether on the inner lips (labial fold mucosa), there were ulcers with diameter about 1 cm. The laboratory test was within normal limits, except the LED was 40 mm/hour (n:&lt;15). The diagnosis was Recurrent Aphthous Stomatitis (RAS) for the ulcer and Exfoliative cheilitis for the cracking lips. The treatment he received was a gargle liquid, topical corticosteroid and supplement. The skin’s disorder was revered to the skin and genital disease department, for further management. As a dental general practioner, had to be very careful and familiar for every changes that may be occur both in the outer or inner oral cavity. Other disorder that need refferal, had to be done with team work, to the colleague from the right connection.</p>

IntroductionHypokalemia can induced Psychotic symptoms even before the physiological manifestations appears. An association is established between hypokalemia and acute psychosis in this case.AimTo Identify the clinical, physiological and metabolic risk factors for all the patients presenting with psychiatric disturbances.ObjectiveTo do medical and base line laboratory investigations to all the patients presenting with acute psychiatric disturbances.MethodA 74 years old gentleman assessed in emergency department following emergency refferal by a general practioner, for an acute psychotic decompensation and suicidal ideation. Patient had a histroy of psychotic episode 30 years ago with one week hospitalization and six month priscription of psychotropic medication. Patient had a relatively stable functioning without any further contact with the psychiatric services till the current episode. After psychiatric assessment patient was discharge home with a priscription of olanzapine 2.5 mg and citalopram 10 mg, for further assessment and investigations by psychiatry of old age.ResultsOne week later patient presented to Emergency Department with shortness of breath and chest dyscomfort. Detailed medical and laboratory investigations revealed Hypokalemia with potassium level at 2.0 mmol/L.The patient showed a prompt improvement in psychosis following intravenous potassium replacement.ConclusionThis case highlights the importance of thorough medical and laboratory evaluation when assessing a psychotic patient. A narrow focuss on psychosis and ignoring organic and metabolic factors can be potentially dangerous to a patient.
