Wilson’s disease is one the rare autosomal recessive disorders of copper metabolism due to mutation in ATP7B gene located in chromosome 13. The mutations of this gene cause accumulation of copper in different tissues such as brain, liver, and eyes. The clinical presentation usually reflects this tissue distribution and varies from asymptomatic patients to those with hepatic or neuro-psychiatric manifestations. Here, we report an interesting case of Wilson’s disease which presented with mild persistent hemolysis leading to pre hepatic and post hepatic jaundice. He also had hepatocellular jaundice due to liver injury.

Most people have become used to spicy fast-food day by day. This has taken human beings far away from nature. Nowadays alcohol consumption is also increasing day by day. Ayurvedic texts have mentioned hepatocellular jaundice as Kamala. Due to frequent intake of Pittakara Aahara (spicy and hot food), it leads to vitiation of Pitta Dosha and Virechana1 (purgation) is the first line of treatment for Pittadushti. Ayurvedic line of management Ac- cording to lolimbraj (Vaidya jivana)2 i.e., Nasya therapy and According to Chakradatta Anjana3 therapy in addi- tion with above mentioned take 3 gm Triphala Churna with Lukewarm water at bedtime for Koshta shuddhi. Nasya and Anjana both are successful in Kamala by removing toxic waste from the body, and by correcting Agni (digestive fire). In our ancient classics, single drugs along with compound drugs have been mentioned in Kamala. These drugs have Kamalahara properties. This results in better circulation and nourishment of the organs and the diseases will subside. Kamala is a disease caused by an overabundance of Mala Ranjak Pitta. Nasya cleanses and energies the tissues and organs of the head and neck. Shodhan Nasya is a form of Nasya that promotes secretions and removes toxins from the body. Devadali Phal Nasya is a form of Shodhan, particularly Rechna Nasya, that causes accumulated Mala Ranjak Pitta to be excreted through the nose. Nasya is referred to as "Nastah Pracchar- dan" by Charaka4. It means Nasya is shodhan karma, which explains Nasya's position at the systemic level and why sr. bilirubin levels fall. Dronpushpi is Doşakarma Kaphavātaśamaka, Pittasamśodhaka Property.It is having Katu, Lavaņa, Madhura Rasa. It is laxative, angmintic, stimulant and febrifuge. The swarasa of this herb is ap- plied as a collyrium (natrănjana) in case of jaundice. Keywords: Jaundice, Kamala, Pitta Dushti, Nasya, Anjana, LFT.

Kamala is a type of Liver disorder mentioned in our classics. It is caused by vitiation of pitta with the signs and symptoms like Haridra netra (yellow eyes),  twak nakha (yellowish skin and nails) and Dourbalya (weakness), based on these clinical features, it can be well understood under the purview of Jaundice. The condition where yellowish discoloration of skin and sclera of the eyes due to high level of serum bilirubin is known as Jaundice.  The pitta present in the body aggravates due to specific diet and life style, attains sthana samshraya in koshta due to kha vaigunya, further causes dagdhatwa of Rakta and Mamsa this condition is known as Koshtashirita Kamala. A case presented with  Peeta Netrata, Peeta Varnyata Mutra, Aruchi, Agnimandhya etc was diagnosed as Koshtashrita Kamala it was intervened successfully with  Shodhana and Shamana oushadi with proper pathya  for a period of 11 days. The effect of treatment was assessed based on improvement in clinical signs and symptoms along with Liver Function Test. There was significant improvement in the condition by 60% thus Kamala can be well managed by Ayurvedic intervention.

Kamala is a Vikar of the pittapradoshja. The symptoms are analogous to those of jaundice. Hepatocellular jaundice is an infectious condition which affects the liver.Due to lifestyle modification, society can't follow these concepts about ‘Aahar’&Vihar’(Diet and behavioural regime)(e.g. fast food, Chinese style food, late-night sleep and day sleep. So now the maximum people in a day are going to suffer from GIT problem. Pittakar-ahar likes Vada-Pav, highly spiced food and propensity to consume alcohol is also rising gradually. Ayurveda has historically been highly proficient and has been treating liver disorders for centuries, and the drug toxicity tends to be lower than conventional medicine. Therefore the case study, like NityaVirechana, is based on Ayurvedic Medicine and Purification Therapy (Panchakarma Procedure). Acharya Charak stated the Classical Virechana is management, but the patient was lean, and his strength was not as much of, so incapable of performing classical virechan. His body structure and his strength and working pattern kept in mind, we planned Nityavirechan (daily purgation) and got a very good outcome and also saved his time for hospitalisation.

Hepatocellular jaundice is a sign that usually accompanies diseases of liver, the organ that detoxifies metabolites, synthesizes proteins, produces biochemicals necessary for digestion, decomposes red blood cells and produces hormones,[1] hence is constantly under the risk of various diseases. Causes of hepatocellular jaundice are numerous with the most common being viral hepatitis, alcohol and drug toxicity. Kamala described among the Pittaja Nanatmaja Vyadhi has Haridra Varna of Netra, Twak, Nakha, Anana, Mutra as the cardinal sign, hence analogous to jaundice. The administration of Gavakshimoola Draava for 3 days is described in the management of Kamala. A clinical trial was conducted on 15 subjects and the results obtained were statistically analysed. Statistically significant reduction of Lakshana of Kamala and serum levels of bilirubin with p-value less than 0.05 was noted.

   The Liver is a vital organ involved in the maintenance of metabolic functions and detoxification of the exogenous and endogenous challenges like drugs, viral infections and chronic alcoholism1. Liver diseases occur throughout the world irrespective of Age, sex, religion and race. According to WHO 46% of global diseases and 59% of the mortality is because of chronic liver diseases and almost 35 million people in world die because of chronic liver diseases2. In Ayurveda, liver disorders and there treatment modalities are very well described under the heading of “Kamala Vyadhi”.         In this study, a case report of 70 yr old male having yellowish discoloration of skin, sclera, and urine. Anorexia (Aruchi), Nausea (hrullas), generalized debility (daurbalya), Dyspnea (Shwas kashtata) since 10-15 days. He was diagnosed as Shakhashrita Kamala and treated with classical Herbal and some Herbomineral preparations as described in Samhita. Significant result was found in the above mentioned symptoms.

Modern culture and advanced technology have made life comfortable, but it’s given invitation to many diseases. In fast life style of people are leading irregular eating habits, eating outside has become fashion which increased risk of contaminated food and water. All these etiological factors lead to risk of related disorders. Bahupitta kamla is one of important disease. Jaundice is a condition in which yellowness of skin, sclera, mucus membrane, and excretions occurs due to hyperbilirubinemia and depositions of bile pigments. Jaundice is described as kamala vyadhi in Ayurveda. IN ayurvedic samhita description of kamala is given in detail. The description of hepatocellular jaundice is similar to ayurvedic description of kamala vyadhi. Here a case report of a 30 years male having Bahupitta Kamala (Jaundice) who was treated with ayurvedic medicine and some panchakarma which give effective results with ayurvedic management.

Chronic liver failure is the functional syndrome resulting from cirrhosis. Clinical features of chronic hepatic decompensation include encephalopathy, coagulopathy, and hepatocellular jaundice. Cirrhosis is the final common pathway for a variety of chronic liver diseases and is characterized by fibrosis and the conversion of normal liver architecture into structurally abnormal nodules. There often exists a poor correlation between biopsy findings and the clinical presentation. Some individuals with cirrhosis are asymptomatic and have a reasonably good life expectancy, while others have severe symptoms of chronic liver failure and limited life expectancy.

Despite a boxed warning, postmarketing reports of deferasirox-associated hepatic injury in patients with chronic transfusions are not well described. Hepatic impairment, including failure, has been reported to occur more frequently in patients older than 55 years and in those with significant comorbidities, including liver cirrhosis and multiorgan failure. In this case report, we describe significant hyperbilirubinemia and acute hepatocellular jaundice related to deferasirox in a 7-year-old female being treated for iron overload secondary to chronic transfusions. This report outlines a unique case without preexisting risk factors in which other causes of liver injury are excluded as defined by the Roussel Uclaf Causality Assessment Method, which indicates a probable score of deferasirox causing the injury.
