Abstract Background Resilience is a widely-used catchword in the last couple of years to describe the resistance to psychological strains of life, especially for the healthcare work-force. The promises of resilience to burnout sound great and what we all would want: less health impairment despite stress, higher work satisfaction and last but not least higher work performance. There is research that shows that students and physicians have high emotional distress and low resilience, yet comparably little is known which aspects of resilience are exactly impaired in the upcoming work-force. With our study we investigated the in-depth resilience status of medical graduates from five medical schools within their first year after graduation. In this, additionally to assessing the resilience status as a whole we investigate the answers on the singular items and the relationship of the resilience status with neighboring constructs. Methods In 2018, 1610 human medical graduates from five Bavarian medical schools were asked to take part at cross-sectional Bavarian graduate survey (Bayerische Absolventenstudie Medizin, MediBAS). The response rate was 38,07, 60% of the participants were female. For the identification of the in-depth resilience status we included the 5-point Likert 10-Item Connor-Davidson Resilience Scale, German Version in a graduate survey posted to 5 medical schools and over 1610 eligible participants of whom 610 (60% female) filled out at least parts of the survey. To identify relationships to other aspects we posed further questionnaires. Results The resilience status showed a mean resilience score of M = 37.1 (SD = 6.30). The score ranges from 3.22 (I am not easily discouraged by failure) to 4.26 (I am able to adapt to change). One third of the participants chose not to answer the item “I am able to handle unpleasant feeling”. Relationships to job satisfaction, scientific competence and stress are presented in the article. Conclusions The study shows that the overall resilience status of medical graduates one year after their graduation is rather high, but subjectively they do not feel equivalently resilient for the different aspects they face in their job. Especially, how to handle their emotions seems to be challenging for some of the young physicians. In the article we sketch ideas how to handle the specific training needs the study has identified.

BACKGROUND Telemedicine as a mode of healthcare work has grown dramatically during the COVID-19 pandemic; the impact of this transition on clinicians’ after-hours EHR-based clinical and administrative work is unclear. OBJECTIVE This study assesses the impact of the transition to telemedicine work during the COVID-19 pandemic on physicians’ EHR-based after-hours workload (“work-after-work”) at a large academic medical center in New York City. METHODS We conducted an EHR-based retrospective cohort study of ambulatory care physicians providing telemedicine services during the pre-pandemic, acute pandemic, and post-acute pandemic periods, relating EHR-based work after work to telemedicine intensity (percentage of care provided via telemedicine), and clinical load (patient load per provider). RESULTS 2,129 physicians were included in this study. During the acute pandemic, the volume of care provided via telemedicine significantly increased across all physicians, while patient volume decreased. When normalizing for clinical load (average appointments per day by average clinical days per week), telemedicine intensity was positively associated with work-after-work across time periods. This association was strongest in the post-acute period. CONCLUSIONS Taking physicians’ clinical load into account, physicians who devoted a higher proportion of their clinical time to telemedicine throughout the various stages of the pandemic engaged in higher levels of EHR-based after-hours than those who used telemedicine less intensively. This suggests that telemedicine may not be inherently more efficient than in-person-based care, and may not reduce the after-hours work burden of physicians. CLINICALTRIAL N/a

This study establishes how demanding healthcare work is experienced to be and whether nurses and physicians experience different levels of workload. A meta-analytic review was conducted of 87 studies that reported Task Load Index (TLX) scores for healthcare work. Of these studies, 37 were conducted in real-life settings and 50 in lab settings without real patients. In real-life settings, clinicians experienced a workload with a mean TLX of 49 (on a 0-100 scale). Divided onto staff groups, the mean TLX for nurses was 63, which was significantly higher than the mean of 40 for physicians. Among the six TLX subscales, the main contributors to workload were mental demand, temporal demand, and effort. They were higher than physical demand and frustration. The clinicians experienced their performance – the last subscale – as closer to poor than good in 38% of the studies conducted in real-life settings. The difference between nurses and physicians was consistent across all subscales, except mental demand. Finally, it is methodologically important that TLX scores appeared not to transfer directly from lab to real-life settings. To reduce the risk of errors and burnout, new healthcare procedures and technologies should be evaluated for their impact on workload.

The Netherlands has an unique system of perinatal and postpartum healthcare. Pregnancy care is delivered predominantly by primary care midwives and childbirth services predominantly involve hospital care. The first week postpartum, all women receive daily care from maternity nurses. In addition, hospitals and out-patient clinics offer perinatal mental healthcare. More specifically, ‘POP care’ (psychiatry, obstetrics and paediatrics) was developed to promote multidisciplinary collaboration in this regard. Although clinical practices and government initiatives to improve pregnancy-related mental healthcare work well, they have yet to be fully described and evaluated. The current COVID-19 pandemic has an impact on health services and perinatal mental health.

Telemedicine is the use of information and communication technologies to extend healthcare work to the vulnerable in the rural areas. It is unfortunate that telemedicine is yet to be deployed in sub Sahara Africa where there is acute shortage of medical professionals with many rural dwellers without medical facilities. This paper proposes an electronic Patient’s Case-Note to replace existing manual method so as to mitigate the challenges associated with manual record keeping. The tree theory was used to motivate the information follows which the basis for the theoretical framework for the study also presented is the Cyclic structure that depicts information flow in the system. The conceptual model and the algorithms to implement the model are presented. The Model was implemented and few screenshot presented.

The presence of contamination in the healthcare work environment by one of the types of hazardous drugs, cytostatics, has been found in multiple international studies. Recent studies and guidelines recommend surface monitoring for risk assessment of healthcare professionals’ exposure. The availability of detection techniques is critical to successfully carry out this type of monitoring. The use of new semi-quantitative techniques allows quicker results. The main objective of this study was to determine the existence of hazardous drugs on the working surfaces in different locations of a tertiary hospital using the BD HD Check® semi-quantitative device. The presence of methotrexate, doxorubicin and cyclophosphamide was analysed at 80, 89 and 82 locations in 10, 13 and 11 clinical units, respectively. A total of 251 samples were analysed. The monitoring results were positive for 13.1% of the analysed samples, with 36.3% of the methotrexate samples, 0% of the doxorubicin samples and 4.9% of the cyclophosphamide samples. Mapping the presence of HD in our hospital has allowed us to evaluate the effectiveness of controls established in the hospital to minimise the exposure of healthcare professionals to hazardous drugs. The speed in obtaining results has enabled immediate corrective actions in cases where contaminated surfaces were detected.
