Objective: An economic evaluation of the resources used for mechanically ventilated patients using various measures for the benefits of extending life.Methods: Regression analysis is applied to New York State discharge data for patients under DRG 475 during 1992–96 to predict age-specific survival rates and payments per life saved. Sensitivity analysis is used to compare benefits of extending life associated with different economic values of life with the payments per life saved at each age.Results: Payments per life saved decreased over time, primarily due to reduced reimbursements. Payments exceeded the age-adjusted and the quality-of-life and age-adjusted benefits for all economic values of life at ages 90 and older.Conclusions: Since the benefits of extending life associated with DRG 475 exceed the payments per life saved at most ages, economic evaluations may be best applied with psychosocial evaluations to allocate resources more ethically.

Economic evaluation is becoming an increasingly important part of the evaluation of health and mental health services. Current models for conducting economic evaluation, including cost-effectiveness analysis, cost-benefit analysis, and cost-utility analysis, have great potential for improving the quality of decision-making and for making mental health programs more effective and efficient. This paper presents the basic economic theory underlying the various forms of economic evaluation and provides general guidelines for developing and conducting an economic analysis of a health program.

The process of allocating financial resources is extremely complex—both because the selection of investments depends on multiple, and interrelated, variables, and constraints that limit the eligibility domain of the solutions, and because the feasibility of projects is influenced by risk factors. In this sense, it is essential to develop economic evaluations on a probabilistic basis. Nevertheless, for the civil engineering sector, the literature emphasizes the centrality of risk management, in order to establish interventions for risk mitigation. On the other hand, few methodologies are available to systematically compare ante and post mitigation design risk, along with the verification of the economic convenience of these actions. The aim of the paper is to demonstrate how these limits can be at least partially overcome by integrating, in the traditional Cost-Benefit Analysis schemes, the As Low as Reasonably Practicable (ALARP) logic. According to it, the risk is tolerable only if it is impossible to reduce it further or if the costs to mitigate it are disproportionate to the benefits obtainable. The research outlines the phases of an innovative protocol for managing investment risks. On the basis of a case study dealing with a project for the recovery and transformation of an ancient medieval village into a widespread-hotel, the novelty of the model consists of the characterization of acceptability and tolerability thresholds of the investment risk, as well as its ability to guarantee the triangular balance between risks, costs and benefits deriving from mitigation options.

Objective: To identify children with ADHD enrolled in New York State (NYS) Medicaid and characterize ADHD-associated costs by treatment category. Method: In 2013, 1.4 million children aged 2 to 17 years were enrolled in NYS Medicaid. Medicaid claims and encounters were used to identify children with ADHD, classify them by type of treatment received, and estimate associated costs. Results: The ADHD cohort comprised 5.4% of all Medicaid-enrolled children, with 35.0% receiving medication only, 16.2% receiving psychological services only, 42.2% receiving both, and 6.6% receiving neither. The total costs for the ADHD cohort (US$729.3 million) accounted for 18.1% of the total costs for children enrolled in NYS Medicaid. Conclusion: This study underscores the importance of achieving a better understanding of children with ADHD enrolled in NYS Medicaid. A framework to categorize children with ADHD based on their treatment categories may help to target interventions to improve the quality of care and reduce costs.

INTRODUCTION:Due to their chronic nature and high prevalence, alcohol and cannabis addiction leads to a significant (disease) burden and high costs, both for those involved and for society. The latter includes effects on health care, quality of life, employment, criminality, education, social security, violence in the public and private domain, and traffic accidents. In the Netherlands, a considerable number of people with an alcohol or cannabis addiction currently do not receive addiction care. Cognitive Behavioral Therapy (CBT) is effective as a treatment for both alcohol and cannabis addiction and is widely used in specialized addiction care centers. This social cost-benefit analysis (SCBA) models costs and benefits of increasing the uptake of CBT for persons with an alcohol addiction and for adolescents with a cannabis addiction, taking into account a wide range of social costs and effects (1).METHODS:The method follows general Dutch guidance for performing SCBA. A literature search was conducted to evaluate efficacy of CBT for alcohol and cannabis dependence. In addition, the social costs of alcohol and cannabis addiction for society were mapped, and the costs of enhancing the uptake of CBT were explored. Costs and benefits of increased uptake of CBT for different social domains were modeled for a ten year period, and compared with current (unchanged) uptake during this period. Compliance problems (about 50 percent of clients do not finish CBT) and fall-back to addiction behavior (decrease of effects of CBT over time) were taken into account in model estimations.RESULTS:Per client treated with CBT, the estimated benefits to society are EUR10,000-14,000 and EUR9,700-13,000, for alcohol and cannabis addiction, respectively. These benefits result from reduced morbidity and mortality, improved quality of life, higher productivity, fewer traffic accidents, and fewer criminal activities.CONCLUSIONS:This SCBA shows that not only treated clients but also society will benefit from an increase in people treated with CBT in specialized addiction care centers.

In the Technology for a Quieter America report, motorcycles were mentioned three times, once in terms of standard test methods and twice in relation to community noise. In New York City, motorcycles placed in the top 10 bothersome noise sources identified by residents. Although there are regulated levels that manufacturers must meet, complaints about in-service motorcycle noise persist. To address this issue, a round table meeting was held in August 2012 with a broad spectrum of participants representing manufacturers, regulators, and other interested, knowledgeable engineers. In preparation of the TQA report, a workshop entitled Cost-Benefit Analysis (CBA) for Noise Control found that CBA was being applied in the area of traffic noise mitigation in regard to the use of barriers and/or quieter pavement. To address the particular CBA issues for this topic, a workshop was held in January 2014 with state and federal transportation officials and the research team responsible for the National Cooperative Highway Research Program NCHRP Report 738, Evaluating Pavement Strategies and Barriers for Noise Mitigation. The findings of these two meetings are summarized in this paper.

The Water as Leverage project aims to lay a blueprint for urban coastal areas around the world that are facing a variety of water-related issues. The blueprint is based upon three real case studies in Bangladesh, India and Indonesia. The case of Indonesia focuses on Semarang, a city that faces issues like flooding, increased water demand, and a lack of wastewater treatment. In this report I summarise the different techniques available to tackling these issues. Along with this I provide a cost-benefit analysis to support decision makers. For a short term it is recommended to produce industrial water from (polluted) surface water as a means to offer an alternative to groundwater abstraction. On a long term it is recommended to install additional wastewater and drinking water treatment services to facilitate better hygiene and a higher quality of life.

The research work was conducted for induced breeding of Vietnames Koi (Anabas testudineus) using carpPG extract and Ovaprim hormone at Tasty Fish and Feeds Industries Limited, Melandah, Jamalpur during March to May, 2015. Brood fishes were reared for 4-5 months in the brood rearing pond by providing farm-made artificial feed (containing 32% protein) for maturation before onset of breeding season. Two different hormones i.e., PG-PG (both for male and female), and PG-Ovaprim (PG for male and Ovaprim for female) were used as inducing agents for the purpose of induction of breeding A. testudineus. The broods were induced with PG-PG hormone at the doses of 2 mg PG/Kg body weight of male and 7 mg PG/Kg body weight of female and PG-Ovaprim hormone were used at the doses of 2 mg PG/Kg body weight of male and 7 ml Ovaprim/Kg body weight of female and designated as treatments T1 and T2, respectively. Ovulation rates (100%) were similar in both the treatments, whereas, fertilization, hatching, and survival rates were 78.11±3.24%, 70.22±3.40%, and 51.22±3.96% in T1 and 71.00±3.97%, 60.89±4.20%, and 48.89±4.41% in T2, respectively. Although comparatively better breeding performance was found for carp PG extract but these values were not significantly higher compared to Ovaprim. Therefore, comparing the breeding performance and considering the cost-benefit analysis it is suggested that Ovaprim could be used as an alternative of carp PG extract for the induced breeding of Vietnames koi.Asian J. During the last century there were two distinct and profound changes in attitudes to animals. First, it became widely understood that human activities and anthropogenic changes to the environment present a serious threat to biological diversity. In response to this many programmes to protect habitat and to conserve species have been launched. Second, advances in various fields of science led to a strengthening belief in many societies that a wide range of animals may have the capacity for consciousness and thus suffering. This has led to growing concern for the welfare of animals - the quality of their lives - and to the development of extensive bodies of welfare legislation in many countries. Concerns for species conservation and concerns for individual animal welfare do not always pull in the same direction. Around the world, conflicts are becoming commonplace between those who believe it can be justifiable to compromise the interests of individual animals in order to prevent species extinctions and those who do not. Such conflicts may be addressed and hopefully avoided or minimized through use of an ethical review process in which conservation benefits and welfare costs are carefully identified, considered and weighed in a cost/benefit analysis. A second function of this review process is to ensure that, where the decision is taken to proceed with a conservation programme that may adversely affect the welfare of some individuals, all necessary steps are taken to minimize these threats and their possible impacts.
