From Swallowing to Savoring Emotions

Abstract. This article presents a Therapeutic Assessment case study of a 48-year-old Catholic nun who was evaluated for concerns related to recurrent depression, difficulty relating to others, trouble following through on tasks, and lack of self-care. Maria was given the Rorschach, MMPI-2-RF, Wartegg Drawing Completion Test, and Thurston Cradock Test of Shame (TCTS). Testing results suggested depression, problems regulating emotion, incongruence between external presentation and internal state, and impaired functional intelligence when negative emotions are triggered. Maria’s TCTS protocol indicated a tendency to deflate in the presence of shame or criticism. She used tentative language around emotion, did not access support, and struggled to resolve emotionally charged situations. The TCTS results appeared to access and explain Maria’s core difficulties. TCTS Cards 6 and 8 were selected for an assessment intervention session (AIS) designed to help Maria understand what she does with negative feelings. Maria was asked to tell a story focusing on the main character. Through half-steps and affective scaffolding, Maria identified how she “swallows” her negative feelings and “isolates” when emotions are strong. The authors discuss how the AIS helped Maria access her split-off affect, and understand its relation to her symptoms, poor self-care, impaired follow-through, and relational difficulties.

AbstractFeeding self-care deficit is a person's inability to perform eating activities according to the proper way of eating. Activity Daily Living (ADL) therapy is one of the therapies claimed to be able to increase the patient's independence in improving how to eat. To apply Activity Daily Living (ADL) in patients with feeding self-care deficit to increase self-feeding independence. The research was a descriptive study with a case study approach conducted in the working area of Puskesmas Dukuhwaru, Tegal Regency. It was carried out on 15-17 February 2021. Data were obtained through interviews, observation and physical examination and presented in narrative form. After performed the intervention for 3 days, the results showed an increase in self-feeding independence in both subjects. It described at the third day of the research. Patients with nursing problems of feeding self-care deficit are recommended to provide ADL to increase the patient's independence. Terapi Activity Daily Living (ADL) menjadi salah satu terapi yang diyakini mampu meningkatkan kemandirian pasien dalam meningkatkan cara makan. Dapat menerapkan terapi Activity Daily Living (ADL) pada pasien defisit perawatan diri makan untuk meningkatkan kemandirian makan. Peneliti ini merupakan penelitian deskriptif dengan pendekatan studi kasus, lokasi studi kasus di Wilayah Puskesmas Dukuh waru Kabupaten Tegal dilakukan tanggal 15-17 Februari 2021, data diperoleh melalui wawancara, observasi dan pemeriksaan fisik. data disajikan dalam bentuk naratif. Setelah dilakukan intervensi selama 3 hari didapatkan hasil adanya peningkatkan makan pada kedua subjek penelitian yaitu dari ketergantungan makan pada awal pengkajian menjadi setelah selesai makan pada hari ketiga penelitian. Pasien dengan masalah keperawatan defisit perawatan diri makan dianjurkan untuk memberikan terapi Activity Daily Living (ADL) untuk meningkatkan kemandirian pasien dalam meningkatkan kemandirian.Kata Kunci: Activity Daily Living (ADL); Asuhan Keperawatan Jiwa; Defisit Perawatan Diri Makan

The objective of this case study, was to understand the effect of the discovery of the inner child in the conjugal relationship of the participants of the Course: TRA: Capacitation in Techniques of Self-Esteem Rescue - Caring for the Caregiver. This is a case study of a couple evaluated through an interview, carried out after the experience of the inner child rescue at the beginning of the course, recordings of the experience sharing and the register of observations in the researcher's notebook. To analyze the opinions collected, the methodology used was discourse analysis. The results showed a greater awareness of the interference of the inner child in the conjugal relationship and the need to be more attentive to its manifestations to avoid future conflicts. Therefore, the Care for the Caregiver course offered a learning space for the inner child and self-care, as well as a way to learn how to care for the other.

RESUMO Objetivo: descrever a percepção das mulheres acometidas por Papiloma Vírus Humano (HPV), em relação à sua situação de saúde e aos tipos de práticas para o autocuidado, baseando-se na Teoria do Autocuidado de Orem. Método: trata-se de um estudo descritivo, qualitativo, realizado com 12 mulheres diagnosticadas com HPV, em duas Unidades Básicas de Saúde, no período de setembro a outubro de 2017. Coletaram-se os dados por meio de entrevista semiestruturada contendo as questões norteadoras, e um questionário para fazer a caracterização dessa mulher em relação à faixa etária, ao estado civil e ao nível de escolaridade. Empregou-se o método de Análise de Dados de Strauss e Corbin, para apresentar os resultados. Resultados: levantou-se que a maioria é jovem, solteira e possui Ensino Fundamental. Emergiram-se duas categorias: 1 - Mulher diagnosticada com HPV: conhecimento sobre a patologia e os sentimentos negativos vivenciados após a descoberta; 2 - O HPV e o autocuidado da mulher. Identificaram-se o desconhecimento sobre a patologia e o uso do preservativo como única forma de prevenção. Conclusão: verificaram-se fatores condicionantes para os requisitos de autocuidado por desvio de saúde, tendo o sistema de apoio-educação como conduta para fortalecer a relação entre o enfermeiro e a mulher. Since the Covid 19 broke out, people are all looking for the cure. Many people died from this fat al disease; and cytokine storm is one of the most dangerous complications. For some low-income people, they are not eligible to be tested for Covid; and they might not get the proper medical care. In these cases, self-care procedures, such as exercise, clear the airway, handwashing, wearing mask, taking meds on time, and proper diet become crucial in deciding the prognoses. This research is about a case study of a patient who is not eligible for a Covid 19 lab test; and how she dealt with a severe flu-like disease.

This case study illustrates how family members can assist in the care of teenagers with diabetes but that there are also serious risks actualized by such involvement. In particular, it highlights ethical complications that arise when the role of a family member is changed from ‘parent’ to ‘care provider’. The ‘counseling, self-care, adherence’ (CSA) approach offers a look at the role that family can play to improve these types of care.

In an environment of escalating healthcare costs, chronic disease management is particularly challenging, since, by definition such diseases have no foreseeable cure and if poorly managed typically lead to further, complicated secondary health issues, which ultimately only serve to exacerbate cost. Diabetes is one of the leading chronic diseases and its prevalence continues to rise exponentially. Thus it behooves all to focus on solutions that can result in superior management of this disease. Hence, this article presents findings from a longitudinal exploratory case study that examined the application of a pervasive technology solution; a mobile phone, to provide superior diabetes self-care. Notably, the benefits of a pervasive technology solution for supporting superior self-care in the context of chronic disease are made especially apparent when viewed through the rich lens of Actor-Network Theory (ANT) and thus the paper underscores the importance of using ANT in such contexts to facilitate a deeper understanding of all potential advantages.

Psychoanalytic tradition of relating to an observing ego makes it unnecessary to conceptualize the Internal Self Helper as paranormal. It is not clear how Rosik conceptualizes the ISH with respect to theological considerations and expectations. Underlying assumptions he makes about the patient in his case study fit better with clinical experience and client self-report than with sound theory. How he views his patient is important, that is, whether she is seen as (a) separate selves, entities, “people,” or alters in one body, or as (b) one whole person but with a fragmented sense of herself. The difference in viewpoints raises significant questions. There is danger in accepting a patient's self-report as literal truth, since (s)he may confuse functional with structural truth, thereby further confusing the clinical picture. Patients with abusive backgrounds are apt to internalize some of the abusive attitudes and negative feelings they experienced in their homes while growing up. Are the perceptions of an ISH held by Rosik's patient her own or those she believes her therapist holds or wants to hold? The ISH is an interesting but as yet not well defined or understood phenomenon which calls for ongoing integrative discussion in the professional community.
