Abstract Introduction: SARS-Cov-2 infection can be asymptomatic, greatly impair respiratory function and, sometimes, affect other organs. Gastro-enteric involvement seems to be not so rare and many patients suffered for abdominal pain, anorexia, nausea and vomiting, diarrhoea or jaundice. Case presentation: Here we report a case of a Covid-19 patient who developed a right colon perforation due to a pseudomembranous colitis without Clostridium Difficile toxins finding associated to a necrotising vasculitis. In this patient a PCR test performed on gastric fluid showed SARS-CoV-2 enteric replication. Discussion: Necrotizing vasculitis is an anatomopathological typical findings in Covid-19. It has been documented in several organs and tissues. In our case, evident foci of necrotizing vasculitis with intravascular obliteration by fibrin thrombi with macrophagic cells infiltration are anatomopathological findings of a CD toxins free pseudomembranous colitis leading to bowel perforation. Conclusions: Bowel perforation due to necrotizing vasculitis leading to pseudomembranous colitis could be a SARS-Cov-2 related clinical finding.

Abstract Background Peritoneal dialysis (PD) is a safe and home-based treatment for end-stage renal disease (ESRD) patients. The direct thermal damage of abdominal organs is very rare. Case presentation We report a peritoneal dialysis patient presented abdominal pain and feculent effluent 3 weeks after he instilled hot dialysis solution. In spite of emergency exploratory laparotomy and active treatment, the patient died of septic shock. Biopsy revealed necrosis and perforation of the intestines. Conclusions Delayed bowel perforation by hot fluid is very rare. Standardized performance is of the first importance for peritoneal dialysis patients.

Introduction. Although SARS-CoV-2 infection is more common in adults, many cases have been reported in the pediatric population. Case presentation. An 8-month-old infant from twin pregnancy, confirmed with COVID-19, is hospitalized with nasal obstruction, serous rhinorrhea, rare cough and watery stools. The epidemiological link is known, both the parents and the maternal grandparents of the infant being confirmed with COVID-19. Biologically, he presented with thrombocytosis and discrete inflammatory syndrome, and the lung radiograph did not show any lesions. At the same time, the twin sister was hospitalized, who presented a similar symptomatology, with a negative SARS-CoV-2 PCR test, but the lung radiograph showed specific lesions of COVID-19. During hospitalization, 2 more SARS-CoV-2 PCR tests were performed, but with negative results. The evolution of the twins was favorable under symptomatic treatment, respectively antibiotic and symptomatic treatment. Conclusion. SARS-CoV-2 infection may be asymptomatic, especially in infants and newborns, and has a lower prevalence.

Abstract Background: The implications of the menstrual cycle for disease susceptibility, development, and severity of acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection are largely unknown.Case presentation: We report a 37-year-old woman infected with SARS-CoV-2 who showed a 16-day incubation period. She developed a fever on the first day of her menstrual period, and again on the first day of her next menstrual period after hospital discharge. RT-PCR test results were positive during the first menstrual period before admission, turned negative during hospitalization, and then positive again during the second menstrual period after hospital discharge.Conclusions: This case indicates sex hormones may play an important role in SARS-CoV-2 infection. For women with history of exposure to SARS-CoV-2, the management protocol should include assessment of the menstrual status.

Granulomatosis with polyangiitis (GPA), necrotizing vasculitis of small and medium-sized vessels, is traditionally believed to mainly affect respiratory tract with additional focal kidney involvements as its primary manifestations with a relatively rare annual incidence rate of 20-50 cases per million. Six percent of the affected cases have cardiac involvements; among which, aneurysms comprise the lowest penetrance. By this paper, we aim to cast light on clinical diagnostic and treatment methods of a rare case presentation, a 10-year-old male GPA patient, diagnosed with massive thrombosis at his coronary artery aneurysm. GPA should be considered as differential diagnosis of prolong fever and coronary aneurysms in adolescents.

Large bowel perforation is an acute abdominal emergency requiring rapid diagnosis for proper treatment. The high mortality rate associated with large bowel perforation underlines the importance of an accurate and timely diagnosis. Computed tomography is useful for diagnosis of ingested foreign bodies, and endoscopic repair using clips can be an effective treatment of colon perforations. We herein describe a 78-year-old man with sigmoid colon perforation caused by accidental swallowing of a jujube pit. The jujube pit had become stuck in the wall of the sigmoid colon and was successfully removed by colonoscopy, avoiding an aggressive surgery. As a result of developments in endoscopic techniques, endoscopic closure has become a feasible option for the management of intestinal perforation.

Objective On 30 January 2020, WHO declared that coronavirus disease 2019 (COVID-19) outbreak has met the criteria for a Public Health Emergency of International Concern. Here, we report the clinical features and outcome of the first two cases of COVID-19 in a university hospital in Turkey, who were housemate internists. Case Presentation Our first patient was a 28-year-old female who works as a resident in the Internal Medicine Department of a 1200-bed, tertiary care University Hospital. Her initial symptom was fatigue. She developed dry cough, nausea, and myalgia. She had no fever, no shortness of breath, or any other complaints. As the 2019-nCoV PCR test was positive, oral hydroxychloroquine and azithromycin were commenced. After the positive PCR test of the first patient, we screened 85 subjects who had unprotected exposure to her; most of them were her colleagues and other healthcare workers. Test results of all but one were negative. The only person with a positive test result was the 35-year-old female who was the housemate and also a colleague of the first patient. Discussion These two cases showed us the desperate situation of COVID-19. To fight an infectious disease that had a high transmission rate, it is important to the infection chain between source and vulnerable subject and transmission route. Unfortunately, to find all infected sources is not possible every time, due to asymptomatic or mild cases, or with low sensitivity diagnostic tests. So, we need to do vigorous isolation precautions and to test more people to end up this pandemic.

Background: A Bochdalek hernia is one of the most frequently occurring congenital diaphragmatic hernias, and it occurs in 1 out of 2,500 births. Clinical cases of adults with symptomatic Bochdalek hernias are very rare. We describe a patient with an incarcerated Bochdalek hernia who underwent emergency surgery and presented with a recurrence after three years. Comparable cases in the literature were reviewed. Case Presentation: A 56-year-old woman presented to the emergency department with acute dyspnea and pain in the epigastrium shortly after gastroscopy. A CT scan showed right sided intrathoracic herniation of the ascending colon and signs of perforation of a hollow viscus. During the operation, a suture repair of a cecal perforation as well as of a diagrammatic lesion was performed. The thorax and abdomen were lavaged and drained. The postoperative course was uneventful. After 3 years, the patient presented with colic pain in the upper abdominal quadrants. A small recurrence of the Bochdalek hernia was observed on a computed tomography scan and was managed with watchful waiting. A systematic search of comparable acute cases of Bochdalek hernias was performed. Conclusion: An incarcerated right colon with a perforation in a Bochdalek hernia is extremely rare; only 11 comparable cases of right sided Bochdalek hernia with acute symptoms are described in the literature. Recurrent Bochdalek hernias are even rarer. As a delay in the diagnosis or a missed diagnosis can ultimately lead to death, adequate imaging is warranted when considering a recurrence of a Bochdalek hernia.

ABSTRACTWe present a rare complication of abdominal liposuction: bowel perforation and necrotizing fasciitis. Because of bilateral lumbar hernia, a 56-year-old woman had caecum and descending colon perforation during lipoplasty. She had septic shock syndrome at her admission. The authors treated this complex wound with several debridement, omental flap, NPWT and split-thickness skin graft. The incidence of abdominal wall perforation with visceral injury is 14 in 100,000 liposuctions performed. There are only 12 cases of bowel perforation in literature but this complication is probably underestimated. Prompt surgical debridement is absolutely mandatory in this life threatening scenario. Lumbar hernia is very rare and should be ruled out before every abdominal liposuction clinically or with imaging modalities.
