Preserving the working population's health is a priority direction of state policy in labor relations, labor protection, and the provision of healthy and safe working conditions by the employer and the state, prevention of occupational diseases. The state's economic growth in a competitive era is mainly due to the level of health and working capacity of the population. This issue has acquired relevance in the COVID-19 pandemic for medical workers. Among these workers, the incidence of the new coronavirus infection COVID-19 is associated with professional duties and entails a high risk of morbidity and mortality. The paper presents the results of a special assessment of working conditions. The author studied the data on the presence of occupational diseases among medical workers in 2019 (according to the data of the Social Insurance Fund of the Russian Federation). On the example of 15 medical organizations of the Russian Federation, an analysis of the working environment factors was carried out. The distribution of occupational diseases was analyzed depending on the harmful factors of the working environment in 2019. Information is provided on the implementation in 2020 by the Social Insurance Fund of the Russian Federation of the Decree of the President of the Russian Federation dated 06.05.2020 No. 313 "On providing additional insurance guarantees to certain categories of medical workers" (as of 07.12.2020). Clinic of the Izmerov Research Institute of Occupational Health and the centers of occupational pathology of the constituent entities of the Russian Federation provided preliminary information on the examinations of professional suitability, studies of the connection of the disease with the profession (including among medical workers) in the Russian Federation in 2020.

The article discusses current regulatory legal acts and clarifications on their application on social support for doctors, paramedical and junior medical personnel of medical organizations, ambulance drivers directly working with patients with a new coronavirus infection (COVID-19). The federal laws, Decrees of the Government of the Russian Federation, and Orders of the Government of the Russian Federation in the field of social support are analyzed. The article contains material on the possible types of medical examinations, systematizes the existing state guarantees for compulsory state insurance for certain categories of medical workers, and the possibility of obtaining the necessary documents from medical organizations and medical and social examination institutions for submission to the Social Insurance Fund of the Russian Federation.

The article covers issues related to compulsory state social insurance, which is one of the mechanisms for ensuring health of employees in the course of their work. In his article the author states that accidents and occupational diseases are one of the biggest threats to the health of the working population of Ukraine. The author of the article emphasizes the need to reorient the institution of compulsory state social insurance to strengthen prevention of accidents and occupational diseases, which, accordingly, has lower economic costs than compensation for workers who have suffered any health damage. He also emphasizes the need to finance preventive measures by the Social Insurance Fund, which is not currently implemented. The article emphasizes importance of updating the list of occupational diseases that occur due to occurrence of modern harmful production factors that harm health of the employees. Currently the list of occupational diseases approved by Ukraine is missing the modern production factors related to digital technologies and psychological risks that are the causes of occupational diseases of the employees. The author also states establishment of the same number of social contributions without taking into account working conditions as another shortcoming of the legislation in the field of compulsory social insurance. The author emphasizes the need to establish occupational risk classes, according to which contributions to the compulsory state social insurance should be paid, which will depend on the occurrence of harmful and dangerous factors in the workplace, number of accidents and occupational diseases. In this regard, the author suggests providing a system of motivation of employers, which provides for the possibility of employers to pay lower contributions in connection with creation of favorable working conditions and reduce accidents and occupational diseases.

Aim. To study the indicators of industrial injuries and occupational morbidity in the Russian Federation and the effectiveness of measures for the professional rehabilitation of injured workers in the context of the implementation of state programs aimed at enhancing the labor force and employment. Methods. The analysis of statistical indicators on the number of workers, insured and injured at work from 2007 to 2018 and the results of measures for victim rehabilitation between 2014 and 2019, using data from Rosstat, reporting form No. 7 social security and open data of the social insurance fund. Descriptive statistics were used in the analysis. Results. The level of employment of disabled people planned by the Government of the Russian Federation by 2020 at the level of 40% in relation to people who are injured at work has not been achieved. At the same time, about 80% of the injured person had mild and moderate dysfunctions, about 50% were of working age. Against this background, employment at the level of 0.2 to 6.6% per year indicates the insufficient effectiveness of vocational rehabilitation. In most EU countries, where the return to work rate for work-injured people exceeds 90%, the employer and the insurer jointly participate in the professional rehabilitation of the injured, applying and financing all the necessary organizational and rehabilitation measures. In the Russian Federation, measures for occupational adaptation, maintenance of employment and the creation of special jobs are not subject to reimbursement from insurance funds, only educational events are financed. An employer who has caused harm to the health of work-injured is not economically interested in restoring his professional status. The authors encouraged to amend the regulations of the Russian Federation to ensure financing of the entire range of measures for vocational rehabilitation and the economic interest of employers in restoring the occupational status of work-injured people. Conclusion. Despite the favorable decreasing trend of industrial injuries and occupational morbidity at the enterprises of the Russian Federation over the past 12 years, the number of industrial injuries with permanent disability remains high, and the effectiveness of their vocational rehabilitation is low, which requires an expansion of the list of measures for vocational rehabilitation financed from the social insurance fund and ensuring the economic interest of employers in restoring the professional status of work-injured people.

The subject of this article was the issues related to the protection of the rights of citizens in the field of compulsory social insurance in the conditions of digitalization. It is shown that in order to ensure effective and safe conditions for the use of digital technologies, it is necessary to include universal basic approaches in the legal regulation. Currently in healthcare of the Russian Federation created a system of measures to combat the harmful effects of industrial noise on workers. However, the level of specific and nonspecific morbidity rates the effect of industrial noise is not reduced, and the incidence of occupational diseases tends to increase. This is due to several reasons, including a large number of sources of noise, not enough high quality of medical examinations, lack of and low effectiveness of means of individual protection from noise and others. Noise is one of the leading places among the harmful physical factors in the Armed forces of the Russian Federation. Features noise resulting from the operation of military equipment and armament is its high intensity, intermittent nature, the presence in the spectrum of low and infrasonic frequencies. Noise exposure leads to the development of diseases, primarily of the organ of hearing, increase in total morbidity and the reduction of military-professional health. In the normative documents of the military medical service the noise is not identified as a harmful factor leading to the development of occupational diseases that were not fully developed the issues of professional selection and medical examination of noise pathology. System noise control should be comprehensive and include organizational and technical measures, special assessment of working conditions, monitoring of noise sources, the presence and correct application of means of protection against noise professional selection, clinical monitoring, medical examination, treatment and preventive measures. Existing in the Armed forces of the Russian Federation the system of measures for the fight against harmful impact of noise on military personnel requires revision in accordance with the existing state legislative framework.
