In order to effectively prevent coal mine accidents, we selected the most serious type of accident in coal mines—gas explosions—as the research object. Based on the accident causation model (24Model), we propose an action path and analysis steps of accidents caused by different employees in the organization. A gas explosion coal mine accident was analyzed using the 24Model and the proposed action path, and 12 unsafe actions, 3 unsafe states, 4 habitual behaviors, 10 safety management systems, and 10 safety cultures were obtained. Case analysis results show that by using the 24Model and path analysis the proposed effect can help employees to clearly identify the cause of the accident, to better understand the logical relationship with the causes of the accident, improve the effectiveness of training, and effectively prevent similar accidents. The 24Model and the proposed path can be used to comprehensively analyze the reasons for and help to effectively prevent coal mine gas explosion accidents.

BackgroundCoal mine gas explosion injury has caused mass casualties, which have resulted in widespread concern.MethodsIn order to prevent gas explosion injury in coal mine, 24Model is proposed to analyse the risk in this paper. 24Model is a human-oriented and organisation-oriented risk analysis method. Based on the 24Model, we propose a general procedure for analysing the causes of injury within the organisation.ResultsA coal mine gas explosion case was analysed using the 24Model and the proposed analysis method, and the evolution of injury and the interaction of various causes was showed, and 6 unsafe conditions, 25 unsafe acts, 13 safety knowledge, 13 safety management systems and 13 safety cultures were obtained.ConclusionCase analysis results show that by using the 24Model and analysis method the proposed effect can help employees to clearly see the evolution and identify the causes of the injury, to better understand the logical relationship with the causes of the injury, improve the effectiveness of training and effectively prevent similar injury. The case study provides a practical procedure for injury investigation and analysis, and thus, preventive measures can be made according to the various causations at different levels.

According to the gas explosion theory and the investigation of occurred gas explosion, the basic events that lead to coal mine gas explosion are summarized and the fault tree is worked out. Besides, the fault tree analysis is used for qualitative analysis. The minimal cuts sets, minimal path sets and structure importance are solved. Accordingly, the reasons of gas explosion are obtained, which can provide references for the prevention of coal mine gas explosion. The study showed that: the minimal cut sets of gas explosion is much(165) and the minimal path sets is small(4), and it is the major dangerous and harmful factors in coal mine. It that gas concentration must be strictly controlled and all sources of ignition should be eliminated is the basic and fundamental way to prevent mine gas explosion.

This paper statistically analyzes the large coal mine gas accidents in China since 2006, and points out that explosion and gas outburst are the main reasons of gas accident, and focus in the future should be put on those serious accidents caused by gas explosion. Death toll of large gas accidents in 2009 is equal to that in 2008, which illustrates the importance of safety input in order to further improve the coal mine safety situation in China. In order to raise consciousness of enterprises on safety input, this paper puts forward the idea of integrating the value of life casualties’ loss into the analysis of loss in coal mine accidents and builds an optimization model of optimization of safety input based on maximization of economic benefit and thorough analysis of safety input, economic losses caused by coal mine accidents and loss of human life. With reference to foreign practices and related research, the value of life for each person is finally set from 5 million to 30 million. Finally it is proved through case study that the consideration of value of life is vital to improve the level of safety input and reduce mortality. Since 2005, the number of deaths caused by gas accident many years occupied the first coal mine accident in major categories. Therefore, the gas accident occurred in recent year statistical analysis, to take specific measures for improving the level of China's coal mine safety is important.

China’s coal mine production situation is grim and various types of accidents occur frequently, and gas explosion accidents are the highest incidence of coal mine accidents. The authors selected 200 gas explosion accidents of coal mine enterprises in recent years, and extracted a large number of workers’ unsafe behaviors. Meanwhile, four working types related to gas explosion accidents were obtained, namely ventilation, gas prevention and fire extinguishing, blasting, and electrician. This article listed some influencing factors of unsafe behaviors and corrected the probability of unsafe behaviors. In addition, a probabilistic risk assessment model was established, and the Monte Carlo method was used to analyze the risks caused by unsafe behaviors of various working types. The results show that the risk of unsafe behaviors caused by the ventilation working type is the highest, followed by gas prevention and fire extinguishing, and finally blasting and electrician. This paper studies the influencing factors of miners’ unsafe behaviors from the perspective of behavior, guarantees effectively the safety management of coal mine enterprises, and lays a foundation for studying unsafe behaviors related to coal mine gas explosions.
