Tower crane are widely used in construction and installation in our country, especially in high-rise building, tower crane is indispensable construction machinery. But the tower crane accident happen high frequency. This essay based on the accident tree analysis methods, combined with tower crane accident type which establish accident tree model, and then resolve the minimal segmental sets, analysis the accident cause factor, According to the qualitative analysis and quantitative analysis of fault tree analysis, we can put forward some prevention ways and control measures to the tower crane accident in those aspects of unsafe actions of the human beings, insecure status of objects and the defects in safety management.

Cutter suction dredgers (CSD) play a major part in the field of dredging engineering in harbors, fairways, and land reclamation. CSD can continuously mine and transport the slop with good quality while CSD can tackle many types of soil. However, even a component failure, which is connected with parts and components of CSD, can result in no working. Thus, it affects the production capacity and efficiency of dredgers seriously. It is necessary to analyze the failure probability of CSD. The method of Fault Tree Analysis (FTA) is applied to analyze the failure probability of CSD. The method uses logical reasoning and builds a fault tree model by analyzing the relationship between the basic, intermediate and top events. Based on fault tree model of a variety of events " and " " or " relationship, we can obtain minimal cut sets of that cause the event to failure. Then, get the basic probability of the event. Finally, we can calculate the probability of the top events. Under experiments, the FTA method can accurately calculate the failure probability of CSD, and deduce in reverse order the key events that cause the top event. The study on the key events can reduce the failure probability of CSD, enhance its reliability and safety, and improve the efficiency and productivity indirectly. It will also lay a foundation for product configuration optimization.

In recent years, with the rapid development of petroleum, chemical and energy industry around the Yangtze River, the variety and quantity transportation of dangerous chemicals has also dramatically increased through Yangtze River. This paper applies fault tree analysis method of specific LPG to obtain problems existing in the Yangtze river water transportation safety management and according to these series of problem, it puts forward some relevant management measures, hoping to minimum the possibility and damage of potential risk accident so as to the risk control measures more scientific, strict and meticulous.

<p><em>Building a construction project is an activity which contains many dangerous elements.  It causes the construction industries have bad record in the terms of occupational safety.  Therefore, occupational safety is an aspect that must be improved any time because it is a very complex problem which covers issues in the terms of humanity, legal aspect, accountability and the image of the organization itself.  This study aims to (1) Determine the accuracy level of the Occupational Safety Management System Application in the Service Building (physical) Construction Project in Tidar Regional Hospital (RSUD) Magelang, (2) Identify and analyze the potential risks of work accident by using Fault Tree Analysis (FTA) method and planning mitigation to reduce the scope of construction in progress. This study is a quantitative analysis where the research is conducted by collecting the primary data through interview and observation, meanwhile the secondary data is in the form of data from the project itself. FTA is used to investigate the potential of workplace accidents by analyzing the direct causes to the basic causes of the accident itself.  The results of the study show that (1) The accuracy level of the application of Occupational Safety Management System which has been applied has an assessment rate of 97.29% and it is included in the level of satisfactory assessment.  (2)  The results of the analysis using FTA obtained several incidents that potentially can cause workplace accidents, incidents such as the workers who do not wear personal protective equipment, workers who act carelessly, and the lack of work experience are the frequent cases in the basic FTA incidents. </em></p>

The success rate of domain name resolution has a direct influence on the service of DNS. Analytical performance of DNS server is the key to measure the satisfaction degree of users when they access to the network. This article establishes the dependence model for the domain name server. In order to get DNS fault model and analytical model, this article uses Fault Tree Analysis theory to describe the relationship of tree basic events and target events of fault tree of dependency. On the basis of fault tree model, dependencies of domain names are qualitative analyzed, including number of sets and element composition of fault model and analytical model. This study provided a theoretical basis for DNS dependencies and technical support for the DNS vulnerability analysis. It has a great importance for domain name system security.

In order to achieve effective stress management in rock drivage team, this paper use fault tree analysis method to build fault tree model for the stress management of rock borehole-blasting drivage team. Under this model, the stressor of rock drivage team is identified, the logical relationship between the stressor and work of errors in rock drivage team is analyzed, the corresponding recommendations for the stress management of coal companies are provided.

The composition and function of vehicle charging system are analyzed, and basic theory of fault tree analysis method is introduced. The qualitative analysis of its reliability is carried out. Fault tree model of vehicle charging system is built, the minimum cut sets are solved and the key parts are found out. Through quantitative analysis, the occurrence probability of top event and importance of bottom event are solved, which provides theoretical basis for fault diagnoses of vehicle charging system, saves time of fault diagnoses and reduces unnecessary disassembly.

Abstract Technological progress, as well as the development of the sciences on occupational health and safety, increased safety and improved working conditions in enterprises of even the most onerous industries. To maintain the required level of safety, preventive actions are taken to identify all hazards in the workplace. In practice, many methods are used to identify hazards depending on the type of technological processes and the types of workstations analyzed. The article presents the advantages of the fault tree analysis method as a tool to support the process safety management in the enterprise. The article presents features of the fault tree analysis method as a tool for better security management in an enterprise. Cause and effect relationships between events in the fault tree schema on selected examples were examined. Indirect and direct causes of accidents and failures in the enterprise have been identified, including human errors (human factor). The use of the fault tree analysis method to support accident and failure risk assessment in an industrial plant was also analyzed.

Fire and explosion accidents occur frequently in tankers because they transport large quantities of dangerous cargo. To prevent fire and explosion accidents, it is necessary to analyze factors that cause accidents and their effects. In this study, factors that cause fire and explosion accidents were classified using the 4M disaster analysis method, and each factor’s effect on the accident was analyzed using fault tree analysis (FTA). First, the unsafe tank atmosphere environment was identified as a primary cause of fire and explosion accidents in tankers, and the underlying causes of these accidents were investigated. The probability of underlying causes leading to primary causes was derived using an expert survey. The results showed that management and media factors had a greater impact on the unsafe tank atmosphere environment than human factors. To prevent fire and explosion accidents, it is necessary to ensure sufficient working and resting times for seafarers and compliance with procedures and work guidelines. A generalization of the results of present and future studies will enable the identification of the cause and preventive measures for fire and explosion accidents in tankers. Furthermore, this will reduce accidents in tankers and contribute to future safety management measures of ships and companies.

Introduction: Factory and Machinery Act (Noise Exposure) Regulation 1989 (FMA [Noise] 1989) has been implemented in Malaysia for nearly 30 years, but noise-induced hearing loss (NIHL) cases is still rising. Fault tree analysis is a top-down approach to analyzing incidences of 'failures', starting with establishing the single top event that will eventually cause NIHL, followed by identification of the contributing factors to the top event which are the immediate or basic events. Through its visual, structural and deductive approach; FTA is able to depict the temporal sequence of events and their interactions in a formal and logical hierarchy. Materials and Methods: Employees with permanent standard threshold shifts (PSTS) underwent further assessment confirming the presence of NIHL. A single common fault tree was constructed based on six cases of PSTS. The top event is the PSTS. Intermediate and basic events were identified and mapped with relevance to the provisions in the FMA (noise) 1989 indicating how failed control measures have resulted in the PSTS cases. Results: The constructed fault tree with its branches illustrated how breach or noncompliance of FMA (Noise) 1989 resulted in the eventual top event (NIHL). Conclusion: FTA provides a standardized perspective of errors within the system in preventing NIHL.
