Objective: The right to health (according to the Article 32 of the Italian Republic Constitution) is financially conditioned; for this reason the National Health System (NHS) has the objective of rationalize health expenditure according to the criteria of efficiency, effectiveness and economy. This paper is an example of rationalization concerning the extracorporeal shock wave lithotripsy (ESWL). Materials and methods: Hospital admissions for urinary stones were taken into account. "Edotto", the database of the Puglia region has identified 23 inpatient admissions during which was performed ESWL. A single operator performed ESWLs with a Storz lithotripter, Modulith SLK. Results: The average hospital stay is conditioned by days "to wait" lithotripsy. In 2014 the hospitalization days "waiting for" lithotripsy were 100. The results were subjected to SWOT analysis and discussed with the Boston Consulting Group Matrix. Discussion: Constant availability of the lithotripter would spare 100 days of hospitalization, amounting to € 88,200.00. This waste of resources corresponds to an additional cost equal to 98.3% on the cost for the rental of the lithotripter. Instead, reducing "unnecessary" hospitalization days would get a saving of 79.3% on the rental cost. It is as if for 46 days of the lithotripter rent were paid 46 days, while for 365 days of the lithotripter rent were paid only 11.8 sessions per year. Conclusions: Rationalization of resources is not necessarily a synonym of "reduction" of resources, but of reduction of waste in the NHS. A good plan is the most important rational basis to get more resources. About the process taken into account it is seen as an investment of € 21,450.00 would keep unchanged the effectiveness of lithotripsy service but would add efficiency and economy (increase of sessions/year, increase in the active mobility, increase in orthopedic treatments) and would drastically reduce the number hospital days (a waste).

<p><em>Abstrak</em> – <strong>CV. Gajah Mungkur adalah salah satu perusahaan yang bergerak di bidang bordir kain. Perusahaan ini mengalami pasang surut penjualan produknya selama dua tahun terakhir. Untuk itu dalam penelitian ini menggunakan strategi seperti analisis SWOT sebagai pendekatan perusahaan menghadapi persaingan bisnis penjualan serta pendekatan dinamika sistem dalam memberikan rekomendasi terhadap keputusan atau kebijakan perusahaan terkait nilai omset penjualan dan nilai profit perusahan yang dipengaruhi oleh faktor ketidakpastian (uncertainty). Berdasarkan hasil pengolahan ANP dengan software Super Decisions, didapatkan bahwa alternatif strategi yang tepat bagi perusahaan CV. Gajah Mungkur untuk melanjutkan proses bisnisnya yang lebih baik adalah dengan cara memberikan promo diskon dan melakukan pemasaran secara online. Berdasarkan analisis output hasil simulasi skenario, bahwa bila perusahaan menerapkan skenario diskon harga produk sebesar 5% perusahaan akan mendapatkan penambahan jumlah penjualan sebanyak 139.258 unit. Kemudian bila perusahaan menerapkan skenario diskon produk sebesar 10% perusahaan akan mendapatkan penambahan jumlah penjualan sebanyak 151.918 unit. Selanjutnya adalah bila perusahaan menerapkan skenario diskon produk sebesar 15% perusahaan akan mendapatkan penambahan jumlah penjualan sebanyak 164.577 unit. Selain itu bila perusahaan menerapkan pemasaran secara online, bila perusahaan menerapkan promosi online salah satunya di online shop shopee maka perusahaan akan mendapatkan penambahan jumlah penjualan sebanyak 174.533 unit.</strong></p><p><em>Abstract</em> - <strong>CV. Gajah Mungkur is one of the companies engaged in fabric embroidery. This company has experienced ups and downs in the sales of its products as per the data for the last two years. For this reason, this study uses strategies such as SWOT analysis as a company approach to dealing with sales business competition as well as a system dynamic approach in providing recommendations on company decisions or policies related to the value of sales turnover and the value of company profits which are influenced by uncertainty factors. The results of this study are based on the results of ANP processing with Super Decisions, it was found that the right alternative strategy for CV. Gajah Mungkur, to continue its better business process, is by providing discount promos and marketing online. Based on the output analysis scenario simulation results, that if the company applies a 5% discount on product price scenarios, the company will get an additional sales volume of 139,258 units. Then if the company applies a product discount scenario of 10%, the company will get an additional sales number of 151,918 units. Furthermore, if the company applies a product discount scenario of 15%, the company will get an additional sales number of 164,577 units. Besides, if the company implements online marketing, if the company implements online promotions, one of which is the online shopee, the company will get an additional sales number of 174,533 unit.</strong></p><p><strong><em>Keywords - </em></strong><em>SWOT, ANP, System Dynamics Simulation</em><em>.</em></p>

Duodenojejunal injuries region at the angle of Treitz are rare, variable etiology and often associated with other serious injuries. In trauma situations with perforations and bleeding, his approach is often difficult. The primary suture, resection / anastomosis and duodenal exclusion are the usual for trauma management techniques in this region. The aim of this paper is to show the initial results of an alternative for injuries duodenojejunal angle (IDJA) by filling a retrospective, descriptive and observational technique, 12 patients operated were analyzed over a period of 15 years, carriers IDJA, age: 34, 11 gunshot wound. 92% of the cases had other associated visceral injuries. The average for the ISS was 29. In all cases located in duodenum duodenojejunoanastomosis II, via the right lateromesenterica upon closing section and duodenum level III was performed. 58% of cases were complicated. Mortality case series were 1 (8%) digestive suture failure colon. Average Hospital stay 26 days. Conclusion: A simple, safe and maintaining gut physiology is proposed technique; with a single anastomosis, located in well-vascularized area and away from bruising and contaminated areas. The complications were pancreatic fistula and digestive suture failure.

Abstract Background Our purpose is to compare the stent patency and clinical outcome of trans-jugular intra-hepatic porto-systemic shunt (TIPS) through the left branch portal vein (TIPS-LPV) to the standard TIPS through the right branch (TIPS-RPV). We retrospectively reviewed all patients (n = 54) with refractory portal hypertension who were subjected to TIPS-LPV at our institute (TIPS-LPV) between 2016 and 2018. These patients were matched with 56 control patients treated with the standard TIPS-RPV (TIPS-RPV). The 2 groups were compared regarding the stent patency rate, encephalopathy, and re-interventions for 1 year after the procedure. Results TIPS-LPV group showed 12 months higher patency rate (90.7% compared to 73.2%) (P < 0.005). The number of the encephalopathy attacks in the TIPS-LPV group was significantly lower than that of the TIPS-RPV group at 6 and 12 months of follow-up [P = 0.012 and 0.036, respectively]. Re-bleeding and improvement of ascites were the same in the two groups [P > 0.05]. Patients underwent TIPS-LPV needed less re-interventions and required less hospitalizations than those with TIPS-RPV [P = 0.039 and P = 0.03, respectively]. Conclusion The new TIPS approach is to extend the stent to LPV. This new TIPS-LPV approach showed the same clinical efficiency as the standard TIPS-RPV in treating variceal bleeding and ascites. However, it proved a better stent patency with lower rates of re-interventions, encephalopathy, and hospital admissions than TIPS through the right branch.

A stone is not only an obstacle, sometimes painful, on the urinary tract justifying an emergency urological gesture to restore their permeability. It is primarily a symptom of crystallogenic pathologies or urinary imbalances of nu-tritional origin whose recurrence is the rule if the cause has not been correctly identified. It is therefore recommended in the patient's interest; analyze the calcul or its fragments to determine its composition and structure, one and / or the other orienting towards the pathology in question. A series of 166 urinary stones were collected nearby the hospitals of western Algeria after urological intervention or spontaneous expulsion and sequential analysis of the nucleus at the surface by Fourier transform infrared spectrophotometry. The male / female ratio was 1.78. The study of the anatomical localization of the stones showed a predominance of the high urinary tract with a rate of 76.9%. 27.3% of the stones were located in the left kidney compared to 25.7% in the right kidney. Analysis of the crystalline composition showed that calcium oxalate was predominant in 65.8% of the calculs and in 58.5% of the nuclei. In all, whewellite was present in 46.9% of cases and weddellite in 18.9%. uric acid anhydrous was the major component of 12.2% of the calculations analyzed. It was present in 23.3% of cases with predominance in subjects over 60 years. Our results show that the lithiasis of the urinary tree in western Algeria tends to evolve in the same direction as that of the industri-alized countries.

The hospital as a business entity in the service sector has its own mission, which is to provide quality services with the achievement of patient satisfaction which is marked by reduced complaints from customers, increased visits so that it shows high company performance. SWOT analysis is a systematic identification of various factors to formulate a company strategy. This research is to find the right strategic opportunity or opportunity, where there is a large market share that allows hospitals to grow and develop. The focus of this research is a strategic plan in an effort to increase hospital income by analyzing a picture of hospital performance in the last 3 years from 2017 to 2019. The study was conducted using a qualitative approach in one of the hospitals in Langsa City. Based on the results of the SWOT diagram, the strategic plan that will be implemented to increase hospital income is to implement standardization of human resources (number of SOP educational task descriptions), cost efficiency, superior services, patient satisfaction surveys and implement efficiency of direct general financing and investment.

KPHP Unit XXVI Belayan Watershed has a management area for production forest and protected forest that has not been optimally utilized. This study aims to develop a business plan strategy in order to optimize the resources of the KPHP Belayan watershed which is oriented towards reducing emissions to achieve sustainable forest management, improving people's welfare, and increasing income. This study use SWOT analysis to determine the right strategy in order to optimize the resources of the KPHP Belayan Watershed. The analysis results show that the commodity business (palm sugar, palm solid sugar, forest honey and kelulut honey, bamboo skewers and carbon trading) by the KPHP Belayan Watershed is financially feasible to be financed. Penelitian mengenai Pengembangan Strategi Pelestarian Budaya di Sanggar Tari Bali Saraswati Yogyakarta bertujuan untuk mengidentifikasi strategi yang digunakan selama ini pada Sanggar Tari Bali Saraswati. Selain itu juga untuk menganalisis faktor internal dan eksternal serta memformulasikan pengembangan strategi yang dilakukan Sanggar Tari Bali Saraswati di Yogyakarta. Metode penelitian mengenai Pengembangan Strategi Pelestarian Budaya di Sanggar Tari Bali Saraswati Yogyakarta dilakukan dalam empat langkah, yaitu menentukan lingkup penelitian yang terdiri dari l okasi penelitian, objek penelitian dan subjek penelitian. Langkah kedua, melakukan teknik pengumpulan data yang terdiri dari dua poin yaitu pengumpulan data primer dan pengumpulan data sekunder. Ketiga, menentukan variabel yang dilakukan melalui dua faktor, yaitu faktor internal (kekuatan dan kelemahan) dan faktor ekstenal (peluang dan ancaman). Langkah terakhir yaitu melakukan analisis data dengan metode kualitatif dengan analisis SWOT. Hasil penelitian pertama, berdasarkan matrik IE Sanggar Tari Bali Saraswati berada pada posisi IV yaitu Stability Strategy. Tidak menambahnya program dan kurikulum pembelajaran yang telah diterapkan sanggar namun, diperjelas lagi dari materi pembelajaran, kurikulum yang ada dan standar kelulusan siswa. Kedua, berdasarkan kuadran analisis SWOT Sanggar Tari Bali Saraswati berada pada kua dran IV, yaitu retrenchment strategy, melepaskan diri dari KPB Purantara atau menentukan sekretariat yang tepat untuk dijadikan secretariat sanggar adalah strategi yang tepat dilakukan. Ketiga, berdasarkan analisis matrik SWOT strategi umum yang diperoleh yaitu menyelenggarakan kegiatan lomba tari Bali setiap semester untuk anak-anak tingkat SD, SMP dan SMA se-Yogyakarta. Bekerja sama dengan panitia upacara keagamaan Hindu (pangempon Pura) yang ada di Yogyakarta. Memberikan kesempatan kepada siswa untuk ikut berpartisipasi dalam kegiatan pengabdian (ngayah) dan event yang mengatasnamakan Sanggar Tari Bali Saraswati. Perlu meningkatkan kesadaran dan rasa memiliki dari pengelola Sanggar Tari Bali Saraswati, anggota KPB (Keluarga Putra Bali) Purantara dan Asrama Bali. Research on Cultural Conservation Development Strategy at Bali Saraswati Dance Studio Yogyakarta aims to identify the strategy used at Bali Saraswati Dance Studio. In addition, to analyze the internal and external factors and formulate the development of strategies conducted by Bali Saraswati Dance Studio in Yogyakarta. The research method on Cultural Conservation Development Strategy at Bali Saraswati Dance Studio is done in four steps, those are determining the scope of research consisting of research location, research object and research subject. The second step, perform data collection techniques consisting of two points: primary data collection and secondary data collection. Third, determine the variables made through two factors, namely internal factors (strengths and weaknesses) and extensions (opportunities and threats). The last step is to perform data analysis with qualitative method with SWOT analysis. The results of the first study, based on the matrix IE Bali Saraswati Dance Studio is in the fourth position of Stability Strategy. Not adding to the program and learning curriculum that has been implemented by the studio but made clear from the learning materials, the existing curriculum and the students' graduation standard. Second, based on quadrant of SWOT analysis Bali Saraswati Dance Studio is in quadrant IV, that is retrenchment strategy, breaking away from KPB Purantara or determining the right secretariat to be used as studio secretariat is the right strategy to do. Third, based on SWOT matrix analysis, the general strategy obtained is to hold Balinese dance activity every semester for elementary, junior and senior high school students in Yogyakarta. In cooperation with the committee of Hindu religious ceremonies (Pangempon Pura) in Yogyakarta. Provide opportunities for students to participate in the activities of devotion (ngayah) and the event on behalf of Bali Saraswati Dance Studio. Need to increase awareness and sense of ownership of Bali Saraswati Dance Studio, member of KPB (Family of Putra Bali) Purantara and Bali Dormitory.

This article investigates and identifies the involvement of stakeholders, analyzes internal and external factors that influence the planning of country border areas, and formulates strategies and programs for developing national border areas. This study used qualitative methods with IFE, EFE, IE and SWOT analysis. Data collection was carried out by observation, interviews, documentation study, literature study, and FGDs. The number of informants was 13 people who are competent and chosen deliberately. The results of this study indicate that the country border area is in a medium internal and external position, meaning that the border area of the country is in quadrant V, the strategy in quadrant V is to hold and maintain. Quadrant V indicates that the border area has developed and needs to be maintained. Therefore, the right grand strategies to be applied are market penetration and product development.Strategies and programs for developing country border areas with the Strengths-Opportunities strategies are mapping and polarization of tourist attractions, maximizing the types and diversity of tourism products in the border areas of the country, and evaluating cross-border festivals. Development programs by Weaknesses-Opportunities analysis are dissemination and training for human resource development particularly for the local people, construction of public facilities, tourism infrastructure, and coordination between stakeholders, and development programs. Meanwhile, by Weaknesses-Threats analysis, namely focus group discussions, seminars, workshops, entrepreneurship training, formal education for the local youths, security and cleanliness of tourist attractions, and regulatory evaluation.
