BACKGROUND The increasing burden of non-communicable diseases (NCDs), which are prevalent in low and middle income countries (LMICs), is largely attributed to modifiable behavioural risk factors such as poor/unhealthy diets and insufficient physical activity (PA). The adolescent stage–recently defined as 10-24 years of age–is an important formative phase of life and offers an opportunity to reduce NCD risk across the life course and for future generations. This protocol describes a portfolio of projects within the Global Diet and Activity Research (GDAR) Network, which aims to support the prevention of non-communicable diseases (NCD), including cardiovascular diseases, type 2 diabetes, and cancers in low and middle income countries (LMICs), with a specific focus on Kenya, Cameroon, South Africa and Jamaica. OBJECTIVE To utilise a convergent mixed methods design to explore exposures in the household, neighbourhood, school, and the journey from home to school, that may influence diet and PA behaviours in adolescents from LMICs. METHODS Male and female adolescents (n ≥150) aged between 13-24 years will be recruited from purposively selected high schools or households in project site countries to ensure socioeconomic diversity of perspectives and experiences at individual, home and neighbourhood levels. The project will be conducted in five sites in four countries; Kenya, Cameroon, Jamaica and South Africa (Cape Town and Johannesburg). Data on anthropometric measures, food intake and PA knowledge and behaviour will be collected using self-report questionnaires and objective measurement in a sub-sample. Additionally, a small number of learners (n=30-45) from each site will be purposively selected as citizen scientists to capture data (photos, audio notes, text, and geolocations) on their “lived experiences” in relation to food and physical activity, in their homes, the journey to and from school, and school and neighbourhood environments, using a mobile application (EpiCollect5). In-depth interviews will be conducted with the citizen scientists’ caregivers to explore household experiences and determinants of food intake and foodways, as well as physical activity of household members RESULTS This project is fully funded by the UK National Institutes of Health Research and conducted by the GDAR Network. Data collection is on-going and analysis will follow once data collection is complete. CONCLUSIONS This project protocol contributes to research that focuses on adolescents and the socio-ecological determinants of food intake and PA in LMIC settings. It includes innovative methodologies to interrogate and map the contexts of these determinants and will generate much needed data to understand the multi-level system of factors that can be leveraged through upstream and downstream strategies and interventions to improve health outcomes

A disease is non-communicable when it is not transferred from one person to another. Typical examples include all types of cancer, diabetes, stroke, or allergies, as well as mental diseases. Non-communicable diseases have at least two things in common—environmental impact and chronicity. These diseases are often associated with reduced quality of life, a higher rate of premature deaths, and negative impacts on a countries’ economy due to healthcare costs and missing work force. Additionally, they affect the individual’s immune system, which increases susceptibility toward communicable diseases, such as the flu or other viral and bacterial infections. Thus, mitigating the effects of non-communicable diseases is one of the most pressing issues of modern medicine, healthcare, and governments in general. Apart from the predisposition toward such diseases (the genome), their occurrence is associated with environmental parameters that people are exposed to (the exposome). Exposure to stressors such as bad air or water quality, noise, extreme heat, or an overall unnatural surrounding all impact the susceptibility to non-communicable diseases. In the identification of such environmental parameters, geoinformation products derived from Earth Observation data acquired by satellites play an increasingly important role. In this paper, we present a review on the joint use of Earth Observation data and public health data for research on non-communicable diseases. We analyzed 146 articles from peer-reviewed journals (Impact Factor ≥ 2) from all over the world that included Earth Observation data and public health data for their assessments. Our results show that this field of synergistic geohealth analyses is still relatively young, with most studies published within the last five years and within national boundaries. While the contribution of Earth Observation, and especially remote sensing-derived geoinformation products on land surface dynamics is on the rise, there is still a huge potential for transdisciplinary integration into studies. We see the necessity for future research and advocate for the increased incorporation of thematically profound remote sensing products with high spatial and temporal resolution into the mapping of exposomes and thus the vulnerability and resilience assessment of a population regarding non-communicable diseases.

PurposeLow-income and middle-income countries such as Tanzania experience a high prevalence of non-communicable diseases (NCDs), including anaemia. Studying if and how anaemia affects growth, placenta development, epigenetic patterns and newborns’ risk of NCDs may provide approaches to prevent NCDs.ParticipantsThe FOETALforNCD (FOetal Exposure and Epidemiological Transitions: the role of Anaemia in early Life for Non-Communicable Diseases in later life) Study is a population-based preconception, pregnancy and birth cohort study (n=1415, n=538, n=427, respectively), conducted in a rural region of North-East Tanzania. All participants were recruited prior to conception or early in pregnancy and followed throughout pregnancy as well as at birth. Data collection included: maternal blood, screening for NCDs and malaria, ultrasound in each trimester, neonatal anthropometry at birth and at 1 month of age, cord blood, placental and cord biopsies for stereology and epigenetic analyses.Findings to dateAt preconception, the average age, body mass index and blood pressure of the women were 28 years, 23 kg/m2 and 117/75 mm Hg, respectively. In total, 458 (36.7%) women had anaemia (haemoglobin Hb <12 g/dL) and 34 (3.6%) women were HIV-positive at preconception. During pregnancy 359 (66.7%) women had anaemia of which 85 (15.8%) women had moderate-to-severe anaemia (Hb ≤9 g/dL) and 33 (6.1%) women had severe anaemia (Hb ≤8 g/dL). In total, 185 (34.4%) women were diagnosed with malaria during pregnancy.Future plansThe project will provide new knowledge on how health, even before conception, might modify the risk of developing NCDs and how to promote better health during pregnancy. The present project ended data collection 1 month after giving birth, but follow-up is continuing through regular monitoring of growth and development and health events according to the National Road Map Strategic Plan in Tanzania. This data will link fetal adverse event to childhood development, and depending on further grant allocation, through a life course follow-up.

ZusammenfassungDie 65. World Health Assembly hat die Bekämpfung nicht-übertragbarer Krankheiten in den Mittelpunkt globaler Aufmerksamkeit und Aktivität gerückt. Da Übergewicht bzw. Adipositas wesentliche Risikofaktoren für einen erheblichen Teil dieser Erkrankungen darstellen, kommt damit der Prävention (aber auch der Therapie) erhöhten Körpergewichtes in der Programmatik der WHO besondere Bedeutung zu. Gleichzeitig führen die hochgesteckten Ziele der WHO in das fundamentale Dilemma, dass es keine Instrumente gibt, die angestrebten Prävalenz- und Reduktionsraten im vorgegebenen Zeitrahmen zu erreichen. Daraus ergeben sich eine Reihe ethischer und sozialer Fragen, unter anderem nach dem zu Grunde gelegten Modell der Adipositas und den impliziten und expliziten Verantwortlichkeiten für ihre Bekämpfung.

Environmental health is referring to the health of people in relation to environmental quality. Due to rapid urbanisation, more people are living in urban neighbourhoods. Urban planning and design aspects including the neighbourhood environment are potentially affecting the human healthy lifestyle and health condition. Thus, the relationship between Malaysian neighbourhood environmental aspects and human health has become the concern of this research. One of the neighbourhood units in Shah Alam city had been chosen as the study area for this purpose. Questionnaire survey had been carried out to examine the health condition and physical activeness of residents. Besides, the perception of residents on the quality of neighbourhood environmental aspects also had been collected in the questionnaire survey. The health condition of residents was measured by non-communicable diseases (NCD) and the physical activeness was measured in duration as well as the frequency of physical exercise activities. The study found that the quality of neighbourhood environmental aspects significantly increased the physical activeness of respondents. The physical activeness was positively associated with human health as measured in NCD (heart diseases and high blood pressure, HBP). It shows that neighbourhood environmental (physical and social) aspects are potentially affecting the healthy life of people living in the neighbourhood.Keywords: Healthy life, Neighbourhood environment, Non-communicable diseases (NCD), Physical activeness, Quality.
