Treatment of keloids is usually challenging, requiring a multimodal approach with no universally accepted treatment modality among the wide range of alternative keloid treatments. Excision of keloid lesion usually eliminates symptoms and it is the main treatment with considerable recurrence rate. Recurrence rate ranges from 45-100% when surgical excision is performed as monotherapy. Furthermore, Recurrent Keloids have a higher recurrence rate after surgery. In this case we discuss a challenging case of young female presented with third recurrence in lobule of the ear with defect necessitated flap reconstruction with concern for possible damage by the flap if radiation was given as external beam postoperatively. Intraoperative electron beam therapy was utilized with high safety and efficacy. To our knowledge this is the first case in the Middle East to use this technique in treating Keloid. Conclusion Treatment of keloids is usually challenging, requiring a multimodal approach. The modern management of esophageal cancer is crucially based on a multidisciplinary and multimodal approach. Radiotherapy is involved in neoadjuvant and adjuvant settings; moreover, it includes radical and palliative treatment intention (with a focus on the use of a stent and its potential integration with radiotherapy). In this review, the above-mentioned settings and approaches will be described. Referring to available international guidelines, the background evidence bases will be reviewed, and the ongoing, more relevant trials will be outlined. Target definitions and radiotherapy doses to administer will be mentioned. Peculiar applications such as brachytherapy (interventional radiation oncology), and data regarding innovative approaches including MRI-guided-RT and radiomic analysis will be reported. A focus on the avoidance of surgery for major clinical responses (particularly for SCC) is detailed.

Polo-Like-Kinase (PLK) 1 activity is associated with maintaining the functional and physical properties of the centrosome's pericentriolar matrix (PCM). In this study, we use a multimodal approach of human cells (HeLa) and zebrafish embryos in parallel to phylogenic analysis to test the role of a PLK1 binding protein, cenexin, in regulating the PCM. Our studies identify that cenexin is required for tempering microtubule nucleation and that a conserved C-terminal PLK1 binding site between humans, zebrafish, and out to cnidaria is required for PCM maintenance through PLK1-dependent substrate phosphorylation events. PCM architecture in cenexin-depleted zebrafish embryos was rescued with wild-type human cenexin, but not with a C-terminal cenexin mutant (S796A) deficient in PLK1 binding. We propose a model where cenexin's C-terminus acts in a conserved manner in eukaryotes, excluding nematodes and arthropods, to anchor PLK1 moderating its potential to phosphorylate PCM substrates required for PCM maintenance and function.

Cachexia is a multifactorial syndrome that presents with, among other characteristics, progressive loss of muscle mass and anti-cardiac remodeling effect that may lead to heart failure. This condition affects about 80% of patients with advanced cancer and contributes to worsening patients’ tolerance to anticancer treatments and to their premature death. Its pathogenesis involves an imbalance in metabolic homeostasis, with increased catabolism and inflammatory cytokines levels, leading to proteolysis and lipolysis, with insufficient food intake. A multimodal approach is indicated for patients with cachexia, with the aim of reducing the speed of muscle wasting and improving their quality of life, which may include nutritional, physical, pharmacologic, and psychological support. This review aims to outline the mechanisms of muscle loss, as well as to evaluate the current clinical evidence of the use of physical exercise in patients with cachexia.

El objetivo de este trabajo es proponer un Modelo holístico multimodal para una lectura crítica del racismo en la prensa escrita. Para ello, desde el Análisis Crítico del Discurso Multimodal (ACDM), usaremos como estrategia teórico-metodológica integradora las aportaciones de Teresa Velázquez (2011) y su modelo semiótico-discursivo; el modelo sociocognitivo de van Dijk (1990, 1997, 2003a, 2003b); el modelo de la semiótica visual de Kress y van Leeuwen (1996) y el modelo intersemiótico de Nikolajeva y Scott (2001). Se validarán las matrices de análisis aplicándolas al llamado conflicto mapuche en Chile para extraer resultados y conclusiones valederas en torno a la representación periodística de las minorías étnicas. Abstract: This work aims to propose a holistic multimodal approach for making critical reading about racism in the written press. In order to achieve this, and taking account the Multimodal Critical Discourse Analysis, we use as theoretical and methodological integrative strategies the contributions of Teresa Velázquez and her discursive-semiotic approach (2011), van Dijk’s sociocognitive approach (1990, 1997a, 2003a, 2003b), Kress and van Leeuwen’s visual semiotics approach (1996), and Nikolakeva and Scott’s intersemiotic approach (2001). The analysis matrices are validated using the so-called mapuche conflict in Chile in order to be able to draw conclusive results and conclusions about media representations of ethnic minorities.

This paper analyses five main slogans and chants performed during the first 3 months of Iraq’s 2019 Tishreen [October] protests. It aims to trace their origin to examine the transformation in the narratives created by each version. Drawing on a social approach to narrative and a social semiotic multimodal approach to communication, I treat slogans and chants as an evolving genre and performance, capable of triggering, constructing, and negotiating a different set of narratives in each adaptation. Such narratives arguably determine their impact. Unlike earlier versions, Tishreen chants and slogans succeeded in conjuring up collective and cross-sectarian narratives that could challenge master political narratives and heighten an Iraqi identity in the first place. It would, therefore, be hard to erase them from memory.

Introduction. In the structure of the global burden of diseases, back pain ranks first in the world in the list of causes of disability. Neck pain is one of the most common diseases of the musculoskeletal system. An integral part of the strategy for the treatment of acute neck pain is the use of original nonsteroidal anti-inflammatory drugs (NSAIDs) with high proven effectiveness.Objective. To conduct a comparative analysis of the therapeutic efficacy of the original drug meloxicam (Movalis) and its generic (Amelotex) in the treatment of patients with acute nonspecific musculoskeletal pain of cervical localization.Materials and methods. The article provides an assessment of the therapeutic effectiveness of the original drug meloxicam (Movalis) and its generic (Amelotex) in the treatment of patients with acute neck pain. Comparative analysis of the results of treatment of 108 patients hospitalized in the Branch of the MC JSC “Admiralteyskie Verfi” aged 18 to 60 years (58 men and 50 women) with a diagnosis of acute dorsalgia of cervical localization (M54.2 according to ICD-10). The average age of patients is 42.5 ± 11.1 years.Results. As a result of the clinical and neurological study and statistical processing of the data obtained, it was found that the use of Movalis in the treatment of patients with acute neck pain was more effective compared to the use of generic (Amelotex). In  the  Movalis group, the  level of  pain  intensity and the  index of  disability after treatment was significantly lower than in the Amelotex group at an equivalent dose of 15 mg per day (p < 0.01). The analgesic effect in the group of patients receiving Movalis was longer. There was a significant decrease in sleep disorders associated with pain syndrome in the group (Movalis) compared with patients of the second group (Amelotex). Conclusions. The absence of therapeutic bioequivalence between Movalis and Amelotex revealed in our study determines the relevance of the choice of drugs for the complex therapy of patients of this profile. A multimodal approach to the strategy of treating neck pain, individually patient-oriented, including the use of effective and safe medicines, physical exercises, manual therapy, is optimal.>< 0.01).
