Objectives: The present study aims to determine the attitudes of care providers including obstetricians, paediatricians and midwives working in perinatal, obstetric and neonatal intensive care unit (NICU) wards of the selected teaching hospitals in Tehran in 2019. In addition, the challenges of providing palliative care from the perspective of these individuals have been examined. Materials and Methods: In this descriptive study, the research population was selected through convenience sampling based on the inclusion criteria. To assess care providers’ attitude toward the perinatal palliative care and the challenges of its implementation, in addition to the questionnaire of demographic characteristics, a researcher-made questionnaire was also used. Results: Most of the care providers (90.5%) believed that parents should be involved in decision-making to select the treatment type. Most of the care providers (90%) believed that the lack of prepared infrastructures is one of the major challenges in providing these types of care. Conclusion: Care providers have almost positive attitudes toward the various dimensions of providing perinatal palliative care, but it has not been properly implemented yet due to the insufficient knowledge of this type of care, the lack of required infrastructures (appropriate conditions in NICUs to provide this type of care, the sufficient number of staff and experts in this field), as well as the health authorities’ neglecting this type of care.

Abstract Objective Caregivers have a great role in palliative care. Considering the difficult process of palliative care, the caregiver undertakes an extra burden with the responsibility of the patient. This study was carried out to evaluate the care burden and preparedness of caregivers who provide care for palliative care patients. Method This study was conducted in the palliative care unit of a state hospital in Turkey between January and May 2019. No sample was selected in the study, and the study was carried out with individuals who were hospitalized in the palliative care unit and who volunteered to participate in the study between the specified dates. The data were collected with Patient Relative Questionnaire Form, Burden Interview, and Preparedness for Caregiving Scale. Results The average Burden Interview score was found as 33.6, SD 13.03 in the study. Patient relatives perceived the patients they cared for as a burden at moderate levels. The Preparedness for Caregiving score average was found as 18.55, SD 6.83. It was found that patient relatives were moderately prepared to undertake the role of caregiver. It was found that care burden decreased as preparedness for caregiving increased and burden increased as preparedness for caregiving decreased (p = 0.001). Significance of results As a result of the study, it was found that caregivers of palliative patients had the moderate levels of care burden and preparedness for caregiving. It was found that as the preparedness levels of caregivers increased, their burdens decreased. Accordingly, it is important that nurses will plan nursing interventions by considering the factors that affect the care burden and preparedness of caregivers.

Khuruj is one of the teachings in Jama'ah Tabligh which requires each member to go outside the area to broadcast amr al-ma`ruf nahy al-munkar within a certain time. They go out for 3 days a month, 40 days a year, and 4 months in a lifetime. The main problem is when the obligation khuruj faced with obligations in the family. Such obligations can cause problems when members of Jama'ah Tabligh can’t balance the obligation to the family in performing khuruj. On the other hand, it is very unfortunate that the lack of coordination and evaluation of care among families of members of the congregation so as to cause negative effects from the community that the tabligh worshipers less attention to families left behind. Therefore, this khuruj program should be studied with the approach of usul al-fiqh (maqasid al-syari'ah), how is khuruj allowed according to syara '. The method used in the study of the analysis conducted by looking at the benefits and harm. Based on this study, it is concluded that khuruj is a real step in the spread of Islamic syiar that not all Islamic organizations are able to implement it. However, the problem of abandoned family requires good coordination in the family, so if the members of Jamaah Tabligh do khuruj must be with the preparation of checking (family must be examined readiness related to intention, stock, physical and family to be left behind) become main and obligatory before doing khuruj so Harm can be rejected.
