Expression of CD74 in Invasive Breast Carcinoma: Its Relation to Nottingham Prognostic Index, Hormone Receptors, and HER2 Immunoprofile

Purpose To study the immunohistochemical expression of CD74 in series of invasive breast carcinomas classified according to their estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (HER2) immunoprofile and explore its correlation to Nottingham Prognostic Index (NPI) and tumor pathologic stage to determine if it has a prognostic value. Methods A total of 160 cases of mammary carcinoma were classified broadly according to their ER, PR, and HER2 expression into luminal, HER2-positive, and triple-negative groups. The NPI was calculated and pathologic stage was recorded for each individual case and cases were classified into different prognostic groups. The CD74 expression was evaluated immunohistochemically and correlated to different prognostic variables. Results The CD74 immunohistochemical expression in invasive breast carcinoma was significantly higher in triple-negative tumors, higher tumor grades, presence of lymph nodal metastasis, higher tumor stages, and higher NPI scores. Conclusions The CD74 might be a useful prognostic indicator predicting poor outcome of patients with breast carcinoma. Its consistent expression in triple-negative breast carcinomas points to the need of further studies to test the possibility if it can be targeted in treatment of breast carcinoma, especially in such groups.

BACKGROUND: Estrogen receptor (ER)-negative breast carcinomas lack the expression of ER and they have no targeted hormone therapies. The androgen receptor (AR) is a newly emerge biomarker. Detecting AR in these tumors may provide a target for future therapies. AIM: The aim of the study is to examine the immunohistochemical expression profiles of AR protein in ER-negative invasive breast carcinomas and to assess the relation between AR expression and the clinicopathologic factors such as age, tumor size, tumor grade, tumor type, immunohistochemical type, lymph node status, and Ki67 expression. METHODS: Sixty paraffin blocks of ER-negative invasive breast carcinoma cases were stained immunohistochemically by AR. Positive expression was defined as ≥1% nuclear staining. RESULTS: AR positivity was detected in 55% of the studied cases. The positive cases were scored by H-score with a median=117, and a range of 3–285 and by Allred score with a median=7, and a range of 3-8. AR is expressed in 60.9% of triple-negative breast carcinoma cases. AR expression was higher in older age, and there were significant positive correlations between the degree of AR expression (AR%, AR intensity, and H-score) and age (p=0.050, 0.007, 0.033, respectively). There was non-significant negative correlation between Ki67% and the degree of AR expression (AR%, AR intensity, H-score, and Allred score). Regarding different histological types, tumor grade, tumor size, lymph node status, and immunohistochemical types, there was no significant difference between AR positive and AR negative cases. CONCLUSION: AR is frequently expressed in ER-negative invasive breast carcinoma; especially in older age, and in a large number of triple-negative subtypes. This may give chance to benefit from future AR target therapy. We recommend further research work on AR expression in the special histologic subtypes of ER-negative breast carcinoma and in the triple negative group.

Objective: To determine the frequency of immunohistochemical expression of androgen receptor in triplenegative breast carcinoma. Study Design: Cross sectional study. Duration and Place of Study: This study included 30 cases confirmed as triple negative breast carcinoma atArmed Forces Institute of Pathology Rawalpindi, from Jan to Jul 2018. Methodology: Anti-androgen receptor antibody was applied and assessed. Positive expression was defined asgreater and equal to 10% nuclear immunostaining. SPSS-24 was used for analyzing data. Results: Out of 30 cases of triple negative breast carcinoma (TNBC), all patients were female. Patients’ agesranged between 21-72 years with a mean age of 46.35 years and a standard deviation of ± 13.4. Androgen receptor expression was positive in 8 cases (27%) of all triple negative breast carcinomas. Out of these androgen receptor (AR) triple negative breast carcinomas; all 8 cases were of histological subtype invasive ductal (mammary) carcinoma, non special type, 7 cases (23%) were of histological grade 3 and 1 was of histological grade 2. Conclusion: Androgen receptor expression is observed in 8 cases (27%) of triple negative breast carcinoma cases. Such patients can be selected as candidates for anti- androgen receptor targeted therapy.

Background: Breast carcinoma is the most common malignancy occurring in females worldwide while in India it is the 2nd most common malignancy occurring after cervical cancer in females. The treatment modalities and prognosis of breast carcinoma depend largely on their hormone receptors (estrogen and progesterone receptors) and human epidermal growth factor receptor 2 (HER2/neu) status. The aim of this study was to correlate the receptor status in invasive breast carcinoma with histopathological grading of the tumor.Methods: This is a retrospective study carried out in the department of pathology, Silchar medical college and hospital, Cachar from January 2019 to December 2020. A total of 69 cases were considered for the study, selected on the basis of inclusion and exclusion criteria. Histomorphology grading in invasive ductal breast carcinomas was done along with the status of estrogen receptor (ER), progesterone receptor (PR) and HER2/neu.Results: The present study comprised of total of 69 infiltrating breast carcinoma cases. Out of all 51% cases were found to be in 4th decade of life. The most common type found in the study was invasive ductal carcinoma (not otherwise specified) seen in 86.96% cases. Maximum 41.67% of cases were histologically classified as grade III. A positive correlation was found between histological grading and immunohistochemistry of hormone receptors (ER, PR) and HER2/neu status.Conclusions: From the present study, it can be concluded that there is a positive correlation between histopathological grade and hormone receptors status and HER2/neu.The estrogen receptor and progestrone receptor were found negative in high grade disease on histological evaluation. Triple negative breast carcinomas have poorer prognosis as they tend to be of high grade.

Background: Breast carcinoma is the most frequent malignant tumor in women accounting for approximately 15% of female cancer deaths. It is the second most common malignancy among women in Nepal. Our objectives were to study the extent and spread of different histological types breast carcinoma in the eastern region of Nepal, to grade and stage the tumors, score the prognosis.Materials and Methods: A cross-sectional descriptive study of mastectomy specimens with axillary lymph node sampling was done for a period of two years. Diagnosis was done using WHO classification. Modified Bloom Richardson score and TNM system was used to grade and stage the tumors. Nottingham Prognostic index was applied to score the prognosis.Results: Out of 31 total cases, the most common histologic type was Invasive Carcinoma of No Special Type (67.74%). The largest tumor size was of 12cm which had poor NPI score. Most tumors were of grade II and T2. Out of 30 cases with lymph nodes, 13 were negative for metastasis pN0, 10 were pN1 and 7 were pN2. Extranodal spread was observed in 6 out of 17 cases with lymph node metastasis and was associated with higher grades and poor prognosis.Conclusion: Higher grade tumors, lymph node metastasis and extranodal extension are associated with higher Nottingham Prognostic Index score.
